今回は、たるみ治療を通じて患者様とともに“理想的な歳の取り方”を歩むために、まずは「たるみの原因」を医学的に解説しながら、その上でウェルエイジングの大切さについてお話しさせていただきます。

顔たるみの原因
顔には図１の黄色の位置に複数の支持靭帯があります。
皮膚・脂肪などが重力で垂れ下がらないように、図２のように靭帯は骨や筋膜などの深い組織と強く固定されています。
加齢と共に靭帯自体が弱くなって支えられなくなると、皮膚・脂肪・筋膜が下垂してたるみます。

図３のように、顔はSector1, 2, 3に分かれており、各Sectorの間には支持靭帯があります(分かりやすくやや大まかに話します)。
つまり間の皮膚や脂肪はハンモック構造となっているのです。
顔の脂肪は、加齢と共にコケる部位と増える部位があります。特にSector2の部分の脂肪は増える部位です。
そのためSector2を中心に頬が弛むのです。

こめかみの骨が萎縮→こめかみのコケ
目まわりの骨が萎縮→目が入っている眼窩というスペースが拡大する→くぼみ目
顎周りの骨が萎縮→フェイスラインの乱れ
一部の支持靭帯は皮膚と骨が連続しているので、骨が萎縮すると皮膚も引っ張られて凹み、顔に凹凸ができます。
Sector2を中心としたたるみと、顔やフェイスラインに凹凸ができる。
これは顔の深い層での変化is.
顔の浅い層での変化としては、真皮のエラスチンやコラーゲン減少によって肌自体のたるみが出現します。
浅い層である肌自体のたるみ＋深い層でのたるみ＝”患者様が悩まれるたるみ” となります。
なので、”たるみ”のご相談をいただいた際は、どの層が関与しているかを判断してアセスメントしています。
