二重の幅よりハム目を治したい場合

睫毛上皮膚切除術の適応です！

睫毛上の盛り上がった皮膚を数mm幅で適量切除して、ハム目の盛り上がりの部分を解消します。

二重幅を狭めたい場合
埋没後の場合：抜糸してから再度低い位置で埋没

全切開後の場合：下のラインで再度切開
元々の二重ラインを超えるまでしっかり剥離して二重の連結構造を離断します
その上で二重を作ります。

二重ラインは高いところで折れやすいので、再癒着防止に２つの方法があります。
・脂肪をクッションにする
・吊り上げ固定もしくは袋とじ縫合により、新しい二重線が折れたままの状態にして癖付けをする

⚠️切開後の幅狭修正で注意すること
・術後のイメージをブジーで表現できない
すでに高いとこで折れてしまっているので、幅狭いところにブジーを当ててもその場所で二重になりません。

・眼瞼下垂手術併用に注意
元々幅広すぎる二重で目の開きは悪くなっています。
幅狭にして二重を作り直すだけで目の開きが少しよくなることが多いです。
そのため、瞼の開きをよくする眼瞼下垂手術を同時に行うと、術後に思った以上に瞼が開きすぎて、
常に目を見開いているような”びっくり目”になってしまう可能性があります。
びっくり目は2回以上の修正回数が必要になるので、避けたい合併症です。
