The most serious complication is postoperative hallux valgus.
I will share my methods for preventing hallux valgus, along with examples of corrections performed at other clinics to illustrate the correction techniques.

In cases where individuals over 50 have sagging skin on the lower eyelids and noticeable dark circles,
Non-incisional bear treatment (”fat removal”) alone will cause skin laxity to worsen post-operatively, so skin excision is also necessary.

The Hamra method is often chosen, which involves excising excess skin while repositioning the bulging fat in the under-eye area downward to smooth out unevenness.
The most frightening complication of that surgery is called ectropion, where the lower eyelid rolls outward like sticking out your tongue.

Reasons for Hallux Valgus and Prevention Methods:
The lower eyelid, in particular, maintains a perfect balance between the forces pulling inward and outward.
The upper eyelid is difficult to flip up, but I think the lower eyelid can be flipped up easily with your finger.
The lower eyelid loses its balance with even a slight disruption in force equilibrium, leading to ectropion or entropion.
① Excessive skin removalThis occurs when excess skin is misjudged and too much is removed. I consider the appropriate amount of skin to remove to be the amount that can be pinched from the lower eyelid while sitting, opening the mouth wide, and looking upward. However, it is crucial to aim for about 60% skin removal initially, with the mindset that any insufficiency can be addressed in a subsequent procedure.
②ScarTaking too much time during surgery, performing unnecessary dissection, or causing severe postoperative contracture can result in outward force and lead to hallux valgus. It is crucial to perform dissection swiftly and efficiently, ensuring thorough hemostasis.
③ The orbicularis oculi muscle hurtsDuring surgery, the orbicularis oculi muscle is incised to access deeper layers, causing partial paralysis of this muscle. Since the orbicularis oculi muscle presses the eyelid against the eyeball, weakened function leads to ectropion. Preventive measures are taken during surgery, such as fixing the orbicularis oculi muscle while pulling it laterally during suturing, or shortening the eyelid width to increase tension if the eyelid's inherent horizontal support is weak.
Mild hallux valgus may occasionally occur for about one month after surgery, but it will improve naturally.
In cases of severe postoperative bleeding, excessive skin removal, or inherently weak lateral eyelid tension, a revision may be necessary to correct ectropion.

Methods for Correcting Hallux Valgus

In this case, we determined that there was no excessive skin removal, the scarring was pronounced, and the lateral tightening was weak.
Make an incision along the previous incision line to release the scar tissue on the lower eyelid.
After that, while partially excising the tarsal plate outlined in blue, the lower eyelid is lifted diagonally outward and fixed to the bone.
The lower eyelid has a hammock-like structure, so the idea is to lift the hammock up and make it taut.
For each case, we determine which factors caused the imbalance in the lower eyelid and make corrections accordingly.

Hallux valgus can be largely prevented by performing appropriate maneuvers during surgery and implementing preventive measures.
It is true that a certain number of patients may experience mild hallux valgus for about a month, but it will improve naturally.
It is rare for a bunion to require correction, but if it does, it's best to have a doctor who can perform the correction handle the initial surgery.
