一番怖い合併症が術後外反です。
私が行っている外反を予防する工夫、他院修正の例を挙げて修正方法をお伝えします。

そもそも表ハムラとは？合併症の外反とは？

このように50歳以降で下瞼に皮膚のたるみがある＋クマが目立つケースでは
切らないクマ治療”脱脂術”のみでは、皮膚のたるみが術後強くなるので、皮膚切除も必要になります。
余分な皮膚を切除しつつ、クマの部分の膨らんだ脂肪を下にずらして凹凸を整える表ハムラ法を選択することが多いです。
その手術の一番怖い合併症は外反と言って、あっかんべーのように下瞼が捲れ上がることです。
目が閉じない、涙がすぐに蒸発する、ためドライアイ症状が強くなり、角膜障害の原因will become.

表ハムラで外反になる理由と予防法：
特に下瞼は、内側に捲れる力と、外側に捲れる力がちょうどいいバランスで釣り合っています。
上瞼はひっくり返すの大変ですが、下瞼は指ですぐひっくり返せると思います。
下瞼の方が、少しの力均衡の崩れでバランスが取れなくなって外反や内反になります。
①皮膚の取りすぎ：余分な皮膚量を見誤って取りすぎた場合に起きます。私は、座った状態で、大きな口を開けつつ、上を見た状態で下瞼の皮膚をつまめる分が適切な切除量と考えています。しかし皮膚切除量は腹6分目を目処に、足りなければ後日追加するくらいの気持ちで行うことが大切です。
②瘢痕：手術に時間をかけたり、無駄な部分を剥離したり、術後拘縮が強くなると外向きの力がかかって外反します。スピーディに、無駄のない剥離で、止血操作をしっかり行うことが大切です。
③眼輪筋が痛む：眼輪筋を切開して深い層を操作するので、眼輪筋が一部麻痺します。眼輪筋は瞼を眼球に押さえつける作用があるので、その作用が弱くなると外反します。縫合時に眼輪筋を横方向に引っ張りながら固定する、瞼自体の水平方向に支える力が弱い場合は瞼の横幅を短くしてテンションを強くする、など手術中に予防処置をしています。
術後1ヶ月程度は軽い外反になることはたまにありますが、自然と改善します。
術後出血が酷かったり、皮膚の取りすぎ、元々瞼の横方向の引き締め力が弱いケースで、修正が必要な外反になることがあります。

外反修正の方法
このケースは皮膚の取りすぎはない、瘢痕が強い、横方向の引き締めが弱いと判断しました。
前回の切開線で切開し、下瞼の瘢痕を解除します。
その後に青線で囲った瞼板を一部切除しつつ、斜め外側に下瞼を引き上げて骨に固定します。
下瞼はハンモック状の構造をしているので、ハンモックを吊り上げてピンと張るイメージです。
症例ごとにどの要因で下瞼の均衡が崩れてしまったのか判断して修正しています。

今回は表ハムラの合併症である外反についてお話ししました。
外反は術中に適切な操作を行い、予防処置を行えばほとんど予防できます。
確かに一定数1ヶ月程度軽度の外反認めることがありますが、自然と改善します。
修正が必要な外反になることは稀ですが、その際でも修正できるドクターに初回手術からお願いするといいでしょう。
