Which Treatment for Your Acne Scars?

Acne scars are more than just skin imperfections; they significantly impact interpersonal relationships and self-esteem.

Redness or crater-like indentations can easily give the impression of looking unhealthy, and there are limits to how well makeup can conceal them.
What I prioritize is not merely achieving beautiful skin, but how a person is perceived and the impression they make on others. Acne scar treatment is precisely the first step in shaping that impression.

3 Types of Acne Scars

Ice pick type:It indents sharply in a V-shape, as if pierced by an ice pick. The indentation is deep.

Cause: Inflammation from acne deforms the pores, causing them to remain open and scar.

Boxcar type:The edges are angular and form square, vertical indentations. The indentations themselves are surprisingly shallow. This is the most common type.

Cause: During inflammation, wound healing did not proceed correctly, resulting in a defect and depression in that area.

Rolling type:It indents smoothly and circularly, as if pressed by a finger. Similar indentations cluster around it, making the surface appear wavy.

Cause: Deep inflammation occurred, causing scarring of the dermis and subcutaneous tissue, which pulled the epidermis downward, resulting in a depression.

 

Treatment Methods by Type

Ice pick therapy:

In some cases, the indentation extends deeper than the dermis into the subcutaneous fat layer, making it difficult for needles like Potenza to reach and posing a challenge for treatment.

If there are only a few: Remove and suture.

When there are many options: TCA cross, Potenza, Dermapen, Picofractional, Juvellook, Tri-Fill, etc.

 

Boxcar-type treatment:

Peeling, Dermapen, Potenza, Pico Fractional Laser, Tri-Fill, and Juvellook are effective.

When the edges of the rim are sharply defined, the sharpness often remains, making it difficult to notice any improvement in the indentation.

In such cases, we improve the appearance by smoothing out the edges with a CO2 laser while lifting the skin from beneath using subcision.

 

Rolling-type treatment:

Subcision and Tri-Fill Pro are effective for cutting the scar tissue pulling the skin downward.

 

Subcision is

The scar tissue is cut by pulling the skin downward with a special needle.

Releasing the pulling force will flatten the acne scar area.

When fat atrophy causes volume loss, injecting hyaluronic acid into the affected area lifts and adds volume while preventing re-adhesion.

Pain, Downtime: Since it's performed under local anesthesia, there is almost no pain. Bruising and swelling may occur for about 3 days.

We recommend a total of two treatments, spaced two months apart.

 

What is Triphil Pro?

 

 

Carbon dioxide is injected beneath the indentation, and a chemical solution is then injected into that space to lift the indentation.

Pain and downtime: Surface anesthesia or nerve block anesthesia minimizes discomfort to a slight prickling sensation. Redness typically subsides within 2 to 3 days.

 

Distinguishing Between Tri-Fill Pro and Subcision

Rigid box car type, shallow rolling type → Tri-Fill Pro

Deep rolling type → Subcision

We recommend using them in this way.
