ニキビ跡は単なる皮膚の凹凸にとどまらず、対人関係や自己肯定感に大きな影響を与えます。

赤みやクレーター状の凹みがあると「不健康そう」という印象を持たれやすく、メイクで隠そうとしても限界があります。
私が重視するのは、単に肌をきれいにすることではなく、その人が周囲からどう見られ、どう印象づけられるか。ニキビ跡治療はまさに「印象形成」の一歩です。

ニキビ跡の3Type

アイスピック型：アイスピックで突いたように鋭くV字型に凹む。凹みは深い。

原因：ニキビの炎症で毛穴が変形し、毛穴が開いたままま瘢痕化。

ボックスカー型：縁が角ばっていて四角く垂直に凹む。凹み自体は意外と浅いです。頻度として一番多い。

原因：炎症時に創傷治癒が正しく行われず、その部分が欠損して凹んだ。

ローリング型：指で押されたようになだらかで円形に凹む。周囲にも同様の凹みが集まっており表面が波打っているように見える。

原因：深い部分で炎症が起き、真皮-皮下組織が瘢痕化して、表皮が下に引っ張られて凹んだ。

タイプ別治療法

アイスピック型の治療：

真皮より深い皮下脂肪層まで凹んでいる場合があり、ポテンツァなどの針が届きにくく治療が難しいタイプです。

数個しかない場合：　切除して縫合

たくさんある場合：　TCAクロス、ポテンツァ、ダーマペン、ピコフラクショナル、ジュベルック、トライフィルなど

 

ボックスカー型の治療：

ピーリング、ダーマペン、ポテンツァ、ピコフラクショナルレーザー、トライフィル、ジュベルックなどが有効。

縁の角ばりが強い場合、角ばりが残ることが多く、凹みの改善が実感しにくいです。

その場合はCO2レーザーで角を落としてなだらかにしながら、サブシジョンで下から持ち上げることで改善します。

 

ローリング型の治療：

皮膚を下に引っ張っている瘢痕を切断するサブシジョンやトライフィルプロが有効です。

 

サブシジョンとは

特殊な針で皮膚を下に引っ張っている瘢痕を切断します。

引っ張る力を解除することでニキビ跡部分が平らになります。

脂肪が萎縮してボリュームが足りない場合は、その部分にヒアルロン酸を注入して持ち上げることでボリュームを足しつつ再癒着を予防します。

痛み、ダウンタイム：局所麻酔で行うので痛みはほとんどありません。内出血、腫れが3日ほど出る可能性があります。

2ヶ月1回ペースで合計2回の施術をおすすめします。

凹みの下に炭酸ガスを噴射し、その空間に薬液を注入することで、凹みを持ち上げます。

痛み、ダウンタイム： 表面麻酔やブロック麻酔で痛みはちくっとするわずかな痛みに抑えられます。赤みは2～3日程度で改善します

 

トライフィルプロとサブシジョンの使い分け

硬いボックスカータイプ、浅いローリングタイプ→トライフィルプロ

深いローリングタイプ→サブシジョン

という使い分けがおすすめです。
