結論：悩んだら裏ハムラです！（ドクターによって考え方が違います！あくまで私の一意見です。）

①：眼窩脂肪の膨らみ　②：tear through 　③：mid-cheek groove

・①の膨らみ
・②③の下にある靭帯の弛み＋②-③間の周囲の脂肪が減ることによる凹み
それによる凹凸が目立つと黒クマが生じます。
①は10歳代でも脂肪量が多いと認めますが、
②、③の溝は、靭帯は皮膚と骨を繋いでいるため特に加齢性変化で骨萎縮が起きると皮膚もくぼんでつくられます。
②は早いと20歳代頃から徐々に深くなり、③は40歳代頃から出現してきます。

①眼窩脂肪の膨らみがある＋②/③の凹みが強い場合、

これが実は一番多いパターンだと思っています。

解決法は2つあります。

◯裏ハムラ法

②③の皮膚を奥に引き込んで凹ます原因となっている靭帯を切って、引き込みを解除しつつ、膨らんだ脂肪をその間に敷き込む

◯脱脂＋脂肪注入

膨らんだ①の脂肪を脱脂で除去

②③の凹みを埋めるように脂肪注入

ではどちらがいいのでしょうか？

結果とも慣れている執刀医であれば綺麗になると思います。

脂肪注入の定着率はヒトそれぞれであり、注入量が多すぎると減らすことは大変なので、適量かやや少なめに注入します。なので②③の凹みを完全に無くすには2回ほどかかる場合があります。

脱脂＋脂肪注入では原因となる靭帯の引き込みは解除されていないので、加齢と共に骨萎縮と共に皮膚が引き込まれると、②③が凹んで来る可能性があるかと思います。

なので私の考えとしては、裏ハムラ法で靭帯の引き込みを解除すべきと考えています。

しかし、②③の凹みが強い場合、それでもボリュームが足りないことがあります。

特に③別名ゴルゴラインは裏ハムラで改善できません。その場合は脂肪注入を併用するとより効果的です。

ただ実際は裏ハムラ単独で十分満足いただけることが多いです。

クマの原理を考えると裏ハムラ法は単独で再発が少ない治療だと考えます。

裏ハムラ法でも埋めきれない凹みが気になれば脂肪注入の併用を検討します。
