Should I get double eyelid surgery or ICL first?

 

ConclusionIn my opinion,

Ptosis Surgery (Surgery to Improve Eye Opening) → ICL

ICL → Full incision/buried suture method 

I think this order is best.

I will explain the reason.

Having eyelid surgery can worsen astigmatism.

In ptosis surgery (surgery to improve eye opening), the shape of the cornea changes due to alterations in eyelid movement itself.

Therefore, there are changes in the astigmatism prescription.

The more difficult the initial opening, the more likely postoperative astigmatism may progress.

After ICL implantation, if astigmatism progresses, you may experience difficulty seeing clearly. However, correcting astigmatism requires replacing the ICL.

It is said that changes in postoperative astigmatism are minimal with buried sutures or full incision techniques.

Therefore, especially for ptosis surgery (surgery to improve eye opening), where postoperative astigmatism is prone to change, we recommend performing the ptosis surgery first, then waiting at least three months for the corneal astigmatism to stabilize before proceeding with ICL implantation.

Use an eyelid speculum during ICL surgery

During ICL surgery, a device is used to keep the eyelids wide open. This can cause the eyelids to be pulled, potentially causing sutures to come loose.

Therefore, for procedures like the sutureless method or full-incision technique that have minimal impact on postoperative astigmatism, we recommend undergoing them after ICL implantation.

We recommend consulting both your scheduled double-surgery doctor and your scheduled ICL surgeon to decide which procedure should be performed first.

If possible, we recommend consulting a doctor who possesses knowledge of both double eyelid surgery and ICL.
