When working in an ophthalmology department, I sometimes receive consultations about eye discomfort after suture removal (though this is self-pay treatment...).
There are various reasons, but here is one factor.
①Postoperative swelling
Swelling of the conjunctiva causes a gritty sensation.
In most cases, the condition will improve naturally with continued use of postoperative eye drops and observation.
②Injuries to the iris or dry eye
During surgery, if the iris comes into contact with sutures or needles, or if the eye becomes dry, scratches on the surface of the iris can also cause a gritty sensation or dryness.
Protecting the eyes with Hyaluronic Acid eye drops or ointment may lead to faster improvement.
If the suture is loose and protruding, or if the suture knot is not properly embedded under the conjunctiva, it will not improve unless removed.
③The conjunctiva on the inner surface of the eyelid has scarred and is rubbing against the pupil.
This occurs particularly when a surgeon unfamiliar with suturing techniques fails to properly embed the suture knot under the conjunctiva.
Even after implantation, if the patient already has allergic conjunctivitis or chronic inflammation of the eyelid, scar tissue may form around the suture knot, causing swelling and discomfort.
Depending on the severity, we will first observe whether the scar shrinks with steroid eye drops.
If improvement does not occur, the scar tissue will be excised. Since buried sutures are often present within it, these sutures will also be removed.
If there is a scratch on the cornea, treat it with Hyalain eye drops or ointment.
④Swelling of the eyelids due to allergic reactions such as giant papillary conjunctivitis
When I turn my eyelid inside out, it's bumpy, and that makes me feel uncomfortable.
Some people have this condition regardless of surgery, especially if they wear contact lenses. Surgery could potentially trigger a worsening of the condition.
If present prior to surgery, it is important to avoid procedures that may irritate the conjunctiva and first focus on controlling the inflammation.
It's difficult to determine without consulting an ophthalmologist. Use steroid eye drops to reduce eyelid swelling.

Items required for diagnosis
This procedure is performed using a slit lamp microscope, which shines light into the eye and magnifies it for observation.
While permanently installed in ophthalmology facilities, they are not available in cosmetic clinics, so I use a handheld slit lamp microscope.

Preoperative assessment is crucial.
Such observation and evaluation are essential even before surgery.
If there is inflammation or injury to the eyelids or pupils, treating it before surgery can help avoid postoperative complications as much as possible.
In cases prone to recurrence, such as conjunctivitis, a full incision that avoids manipulating the conjunctiva may be preferable to suture-based buried suture techniques (and surgery may not be recommended at all).
The same applies to treatments for dark circles, lateral canthoplasty, droopy-eye correction, and ptosis surgery. If there are issues with the conjunctiva or iris, those conditions must be treated first.
