7月頃がピークになる紫外線、もともとあったシミが濃くなりやすいです。
紫外線は細胞のDNAやコラーゲンなどを直接損傷させるだけでなく、活性酸素を発生させます。その活性酸素によってさらに光老化が進行します
その紫外線から守ってくれるのがメラニンです。日に焼けて肌が黒くなるのは一種の生体防御反応の一つなのです。
しかし部分的にメラニン生成が過剰になるとシミ(老人性色素斑)やそばかす(雀卵斑)の増悪に、慢性的な肌の炎症が強く出ることで肝斑が増悪します。
初期の肝斑の場合、患者さん自身が肝斑と自覚していないことが多いです。
秋になって紫外線量が少なくなると濃くなったシミも、ターンオーバーにより少しずつ排出されて自然と元の肌に戻っていきます。
しかしターンオーバーの乱れやメラニン生成がそのまま続いてしまうと、なかなか治ってくれません。
自然に治るのも時間がかかるので美容医療の力を借りましょう！
改善方法
◯日焼け直後”72時間”のゴールデンタイム以内ならまずはこれ！
高濃度ビタミンC点滴
メラニン生成抑制効果、抗酸化作用による活性酸素による炎症改善＋黒色メラニンを褐色化させる直接還元作用
によって肌のトーンをなるべく維持します。
◯日焼け72時間後
炎症がまだあると判断した場合、メソナJでトラネキサム酸やビタミンC誘導体などを直接皮下に導入して鎮静を行います。
その後は原因によって治療が異なります。
シミ(老人性色素斑)やそばかす(雀卵斑)の場合
メラニンが沈着しているので、ターンオーバーを促進させて排出させる事が大切になります。
ピコスポットでメラニンを破壊して外に出す事が最も有効ですが注意が必要です。
日焼け直後だと色素沈着が強く出る可能性があるので、レーザー前3ヶ月の遮光の徹底をお奨めしています。
その上で
をレーザー1ヶ月前から施術後半年使用いただくと色素沈着しにくいです。
そばかすの場合は3回ほど治療が必要なことが多いです。ピコスポットの治療間隔は半年空けてください。
肝斑の場合
肌の慢性炎症が原因です。
鎮静＋メラニン排出＋基底膜強化の３本柱で治療します。
鎮静：
トラネキサム酸500mg/日 内服＋メソナJ
最近はプルリアルの手打ち水光注射も効果的と言われています。
メラニン排出：
炎症をコントロールした上でピコスポットより出力の低いpicotoningを肌全体に照射することで
メラニンを過剰生成させているメラノサイトを刺激せずに、全体的にくすんで見える沈着したメラニンを少しずつ削り取って除去します。
基底膜強化：
表皮と真皮の間には基底膜という仕切り構造があります。基底膜よりも表層の部分でターンオーバーが起きています。肝斑病変部はこの基底膜がもろくなり、本来表皮内にあるメラニン色素が、真皮側に落ち込むと言われています。そして真皮内のメラニンは消えにくく治りにくくなると考えられています。
potenzaは、細かな針で肌に微細な傷を与え、高周波の熱を照射する治療法です。この熱によって基底膜が強化され、基底膜に存在するメラノサイト(メラニンを作る場所)を抑制＋ターンオーバー正常化によって肝斑が改善します。
