直径16cmのこの器具、、、ある手術で、私にとって欠かせないものです。

正式名称”上山式中深部持針器”、脳外科の名医上山博康先生が開発、使用されているもので、細いトンネルの先の細かいものを操作するのに適しています。

脳外科医でない私が、何に使用しているのでしょうか？

正解は裏ハムラ法です。

裏ハムラ法とはクマ治療の術式の一つで、瞼の裏からクマの脂肪を下にずらして改善させるものです。

狭い視野の中で眼窩隔膜という薄い膜をクマの脂肪ごと下の骨膜に結ぶ必要があります。

瞼が持ち上がりにくい患者さんではさらに視野が狭く難易度が高くなりますが、上山式中深部持針器があれば狭い空間でも細かな操作が可能になります。

色々な器具を試す中で、この手術器具に出会いました。

結ぶ糸にもこだわりを持っています。糸の太さ、通す針の長さや曲がり形状でも手術のやりやすさが変わってきます。

”弘法筆を選ばず”と言いますが、いいものが溢れている現在、より良いパフォーマンスを発揮するためにも状況に適した筆を選び抜く力も大事だと考えています。

因みにこの持針器、1本で10万円以上します、、、もちろん自腹です！

先が細いので落として壊れないかいつもヒヤヒヤして使っています。
