Hello, I'm Ryutaro Natsume, an image designer and cosmetic surgeon.

Few people are aware they have melasma.

Some patients report that melasma was first pointed out to them during a consultation using the VISIA skin diagnostic device, which they had sought primarily for concerns about dark spots and skin dullness.

This time, we'll explain melasma treatment!

What exactly is melasma?

Liver spots are,

Melasma refers to light brown pigmented patches that appear symmetrically on both cheeks, temples, and forehead, typically of equal size. Unlike clearly defined age spots (liver spots), melasma has blurred edges.

Chronic skin irritation, ultraviolet rays, and female hormone balance are considered causes.

Symptoms typically persist from the 30s to 40s through the late 50s, and may lessen or disappear after menopause.

Liver Spot Treatment: Start with Skincare and Sun Protection

◯Skincare

It may seem obvious, but surprisingly, many people see improvement simply by adhering to this.

The reason melasma is more common in women is that women tend to rub their skin more often.

The process of applying makeup and removing it, thorough cleansing morning and night—because their beauty consciousness is higher than men's, they tend to rub their faces more frequently.

Use a foaming cleanser, and a small amount—just enough to lightly cover your palm—is sufficient. Using too much means you'll have to rinse your face more thoroughly.

For makeup removal, we recommend milk or gel cleansers that cause minimal friction.

◯Light blocking

Ultraviolet rays cause free radicals to form, leading to skin inflammation and the activation of melanocytes, which worsens melasma.

We recommend using sunscreen as a makeup base and applying sunscreen spray every three hours.

Suppress inflammation

◯ Oral Tranexamic Acid

Take the medication for at least three months.

Plasmin, which promotes the activation of melanocytes—pigment cells—and is involved in melanin production, suppresses the action of prostaglandins to reduce inflammation associated with melasma.

◯Mesona J

It is said to have 300 times the penetration effect of topical application and 70 times that of iontophoresis.

By introducing anti-inflammatory substances such as tranexamic acid and vitamin C derivatives into the skin, chronic inflammation is suppressed.

◯Pullreal

A formulation containing polynucleotides and mannitol that has an anti-inflammatory effect.

It is recommended to administer the product via subcutaneous injection, creating a small blister to introduce it into the skin.

pigment discharge

picotoning

This treatment reduces melanin pigment using shock waves from a low-power laser, making spots, freckles, melasma, and dullness less noticeable.

◯Peeling

This treatment promotes epidermal turnover to expel unevenly distributed pigments and restore a clear complexion.

Basement membrane reinforcement

This is a concept that has been gaining attention in recent years.

Disruption of the basal membrane deep within the epidermis can cause melanin to sink into the dermis, an even deeper layer.

Removing pigment located deep within the skin is not easy, which is why melasma treatment is considered difficult.

Potenza

The high-frequency (RF) energy emitted from the needle tip restores the basement membrane to its youthful, wavy state. This suppresses the abnormal condition where melanocytes—the root cause of melasma—continue to overproduce melanin. Simultaneously, the micro-injuries created by the needles stimulate skin turnover, promoting the expulsion of melasma for improvement.

Summary

We treat melasma using multiple approaches in this manner.

Few people are aware they have melasma.

Many individuals troubled by dark spots also have melasma.

Applying strong energy treatments like Pico Spot to melasma can worsen the condition.
