大事な日なのに目が充血してしまった…よく患者様だけでなく、同業や友人からも相談を受けます。
眼の充血の原因は様々です。
今回はその原因の一部をご紹介します。
◯アレルギー
アレルギー性結膜炎に伴う充血を引き起こすことがあります。
最近だと(9月)ブタクサ花粉が飛ぶのでアレルギー症状が強くなることがあります。
抗ヒスタミン点眼(アレジオンLXなど)を使用することが多いですが、症状が強いとステロイド点眼を使用することがあります。
◯感染
ウイルスや細菌に伴う結膜炎で生じることがあります。
抗菌薬点眼などで経過を見ます。ステロイド点眼を併用することもあります。
アデノウイルスが原因の流行性角結膜炎は非常に感染力が強いので、周囲の方に移さないように感染対策をしっかり行う必要があります。特に夏に多いです。
ぶどう膜炎など、眼の中の炎症によっても充血を起こすことがあり、その場合は黒目近くが特に充血しやすいです。
◯ドライアイ
ドライアイが強いと角膜上皮障害を起こします。
ヒアレイン点眼やジクアス点眼など角膜を保護する涙液層にアプローチする治療が必要になります。
◯コンタクト
コンタクトによって酸素を通す割合が違います。大体安いものは低く、高価なものは酸素透過度が高い傾向にあります。
酸素不足の組織に酸素を届けようと周囲の血管拡張を起こす場合があります。コンタクトの酸素透過度を高いものにすると改善する可能性があります。
コンタクトのフィッテイングが悪くてもごろつきの原因になったり、擦れて充血しやすくなります。角膜形状に合うコンタクトへ変更することで改善することがあります。
コンタクト使用者に多い巨大乳頭結膜炎という強いアレルギー性結膜炎でも同様に充血することがあります。その場合コンタクトを中止してステロイド点眼で治療します。
◯むくみ
二日酔いや締めのラーメンなど食べたと体が浮腫みますよね。
同様に白目も浮腫、充血します。
ステロイド点眼で改善します。血管収縮効果のあるナファゾリンも効果的です。

充血の対応策と言っても原因によって、治療が大きく異なるのがお分かりいただけたと思います。
片眼か、両眼か、も診断に重要なポイントです。
特にぶどう膜炎など眼の中の炎症は緊急度が高いです。自分で判断せず、眼科受診をおすすめします。
その方が充血を改善させる一番の近道です。
