蒙古襞（もうこひだ）とは、上まぶたの内側から目頭にかけて皮膚がかぶさる構造のことです。
アジア人に多く見られる特徴で、目元の印象を決定づける重要な要素です。
まず知っておきたいのは、寄り目・離れ目といった「目の距離の見え方」は、先天的な眼球の位置関係に加え、蒙古襞の有無によっても左右されるということです。
さらに、蒙古襞の張り具合によって“そう見える度合い”が変化します。

■ 蒙古襞が与える印象の違い
蒙古襞は、目元の印象をかなり左右するパーツです。
● 蒙古襞が強い場合
• 少し幼い・あどけない印象
• 目の横幅が小さく見え、離れ目傾向に
• 柔らかい雰囲気になりやすい
● 蒙古襞が少ない／弱い場合（先天的）
• 大人っぽいが、ややシャープな印象
• 目の距離が近づく傾向
• クール／強さのある雰囲気になりやすい
つまり、目頭切開で蒙古襞を切りすぎてしまった場合には、
• 求心顔が過度に強調される
• きつい、または老けた印象に見えやすい
• 顔全体のバランスが崩れ、不自然に見える
といったお悩みにつながることがあります。

■ 蒙古襞形成（目頭切開修正）とは？
蒙古襞形成は、過度に露出した目頭を自然な位置に戻し、印象を整える手術is.
• 目頭切開後の求心顔が気になる
• きつい印象の目元を柔らかくしたい
• 涙丘が出過ぎて違和感がある
こういったケースに適しています。
もともと蒙古襞が少ない方に対しても

● 逆Z法
立体的でナチュラルな蒙古襞を再形成。
寄り目感を自然に和らげるのに適しています。

● V-Y法
ミリ単位での調整に強い。
逆Zに比べて丸みある仕上がりに。
左右差が強い場合にも有効です。
傷跡はほとんど目立ちません。

• 腫れ：3日程度（軽い浮腫は10日ほど）
• 抜糸：7日後
• 洗顔・シャワー：翌日より可

■ 二重の変化について
蒙古襞形成を行うと平行型 → やや末広型に寄る傾向があります。
これは蒙古襞が復元され、二重の内側がわずかに隠れるためです。
そのため、術前のシミュレーションや術者と患者の情報共有が非常に大切です。

■ 目頭の形はメンタルにも影響する
蒙古襞は 0.5〜1mmの変化でも印象がかなり変わります。
目頭切開の修正のカウンセリングをしていて思うのは、その繊細さゆえ患者様の心理面に強く影響していることです。
In particular,「切りすぎた気がする」「人前に出られない」など、このような訴えは非常に多く、修正を急ぎたくなる気持ちが大きくなります。
一方で、「変化が物足りない」という場合は時間をかけて調整する選択肢もあるため緊急性はありません。
厄介なのは、繊細になりすぎて、切りすぎ → 修正 → 切開 → 修正 …と 負のループに陥ることです。
手術前に執刀医とゴールをしっかり共有すること、過度な変化を求めないこと、が最も重要will become.
