前回の記事では鼻中隔延長術とはどんな手術かについて解説しました。
鼻中隔延長では、「どの軟骨を使うか」が仕上がりを大きく左右します。
なぜなら鼻中隔延長は、鼻先の形を作る手術ではなく、鼻先を支える骨組みを作る手術だからです。
骨組みの構造が弱ければ、術直後は見た目が整っていても時間の経過とともに不安定になり、傾いたり後戻りしたりするリスクが高まります。
そのため、軟骨の種類と役割を理解したうえで材料を選ぶことが重要です。

■ 鼻中隔軟骨

鼻中隔軟骨は、鼻の奥にある左右の鼻を隔てる中央の軟骨で、鼻中隔延長の基本となる材料です。
鼻の中から採取できるため、新たな傷を作らずに済み、十分な量が確保できれば硬さと安定性に優れた土台を作ることができます。
ただし、日本人では鼻中隔軟骨が小さい方も多く、量が足りないケースも少なくありません。
また、鼻中隔軟骨は骨組み作りには適していますが、鼻先の丸みや質感を作るには硬さが出やすい素材です。
そのため実際の手術では、
鼻中隔延長は鼻中隔軟骨で行い、鼻尖形成や鼻先の軟骨移植は耳介軟骨を組み合わせる
という構成が基本になります。

■ Auricular cartilage
耳介軟骨は、耳の後ろから採取する軟骨です。
耳の付け根のラインに沿って切開するため、傷は目立ちにくく、切開も2cm程度です。
柔らかく、しなやかさがあるため、鼻先の丸みや質感を自然に整える役割に向いています。
鼻中隔軟骨の硬さを和らげ、仕上がりをより自然に見せるために欠かせない材料です。
なお、耳介軟骨のみを使って鼻中隔延長を行う場合には注意が必要です。
この場合は、左右両方の耳介軟骨を2枚使用することが基本will become.
片側だけでは鼻中隔延長に必要な量と強度が不足し、鼻先の傾きや骨組みとしての不安定さにつながるためです。

■ 肋軟骨

肋軟骨は、肋骨（あばら骨）の一部から採取する軟骨です。
バスト下の付け根に沿って切開するため、傷はバストに隠れやすく、時間の経過とともに目立ちにくくなりますが、体に傷跡が残る点はデメリットの一つis.
However、そのデメリットを差し引いても十分なメリットがあります。
それは、採取できる量が豊富で強度が高く、鼻中隔軟骨が小さい方でも、安定した骨組みを作ることができる点です。
また鼻中隔延長だけでなく、隆鼻術や貴族手術にも同時に使用できるため、近年では鼻全体を肋軟骨だけで作り上げる「肋軟骨鼻フル」として用いられることが増えています。
肋軟骨を採取しても日常生活に大きな支障はなく、術後2週間ほどで運動も可能です。

■ 材料選びで大切な考え方
鼻中隔延長において重要なのは、「どの軟骨を使うか」そのものではありません。
現状の鼻に対して、どの程度の変化が欲しいのか。
そのためにはどれくらいの材料が必要なのか。
この設計に合わせて材料を選ぶことが、長期的に安定した仕上がりにつながります。
鼻中隔延長を行う場合、簡単にまとめると、
「肋軟骨」
→ 鼻の形そのものを大きく変えたり、大幅に高くしたい場合
「鼻中隔軟骨＋耳介軟骨」or「耳介軟骨2枚」
→ 鼻の形はそこまで変えず、大幅ではないがそこそこの高さが欲しい場合
このような認識で良いかと思います。

鼻中隔延長に使われる軟骨にはそれぞれ適性と特徴が異なります。
大切なのは材料そのものではなく、どの程度の変化が必要かに応じて、十分な支持性と安定性を確保できる材料を選ぶことです。
この考え方が、長期的に安定した手術結果につながります。
