Double Buried Suture vs. Full Incision: Which Double Eyelid Surgery Is Right for You?
A dual-specialty doctor with a background in plastic surgery, bridging cosmetic surgery and ophthalmology. At the university hospital, he performed numerous high-difficulty cases, primarily focusing on eye and facial bone surgeries, cultivating precise techniques grounded in anatomy. He prioritizes achieving both aesthetic beauty and functional safety, offering customized treatments around the eyes that incorporate ophthalmological expertise. He actively utilizes not only incisional surgery but also non-invasive procedures like hyaluronic acid injections. By carefully assessing each individual's condition and unique appeal, he provides bespoke treatments that guide patients toward naturally harmonious, well-defined eyes.

Huh? I didn't realize... Is that what you thought? 😏
Many reasons for feeling discomfort after double eyelid surgery include asymmetry and "ham-eye" (※a type of double eyelid where the skin between the crease and the lash line bulges).
Whether it's double eyelid suture or full incision, the result will look natural if the optimal double eyelid is tailored to each patient's unique bone structure.
However, the optimal double eyelid width may not necessarily match the patient's ideal, making thorough preoperative consultation crucial.
As a cosmetic surgeon, while I do propose double eyelids that suit the patient's bone structure, I consider it my mission to strive to bring them closer to the ideal eyelids they have envisioned.
(In cases where it is clearly evident that the surgery would lead to an unfortunate outcome due to an extreme mismatch with the skeletal structure, we may point this out.)
Here, we'll clearly explain the differences and features of each treatment while guiding you on which one is best suited for whom.
👁Double Buried Suture Method
Medical-grade thread finer than hair is used to suture the eyelid to create double eyelids.

◎Benefits:
・Wide parallel double eyelids are possibleFull incision can achieve even the width that would result in a ham-shaped eye.
・Short downtimeSince it only involves threading the suture, tissue damage is minimal.
・Less likely to leave marksOnly a small pinprick that heals within a few days.
・Can be restoredRecommended for those concerned about the final shape
・Short surgery time(10-20 minutes), low price: perfect for beginners
△Disadvantages:
・Not for lifeEventually, when the thread ball comes off, the double layer will loosen.
・Not suitable for those with poor eye openingWhen ptosis is severe enough that non-surgical treatment is difficult, buried suture procedures may further worsen the opening.
・Not suitable for those with thick eyelids or significant skin laxityIf the double eyelid crease doesn't form properly and comes undone with the Buji method, it may come undone quickly with the double eyelid suture method.
To prevent recurrence, orbital fat can be removed through a small incision. However, individuals with significant fat deposits often have thicker skin, making the sutures prone to coming undone. In such cases, I recommend a full incision from the outset.
There are various surgical techniques for the buried method, but I will omit them for now.The greatest advantage of the buried method is that it can be removed and restored to its original state.Personally, I believe that's the case, so I prefer simple rituals.
👁Full Incision Method
An incision is made in the eyelid, excess fat and skin are removed, and the eyelid tissue is used to recreate the double eyelid structure.

◎Benefits:
・High durability:You can draw clear lines.
・Adjustable eye openingEyelid ptosis surgery can improve eye opening. In fact, creating a wide double eyelid crease using full incision alone can sometimes result in poor eye opening. This can be prevented by slightly improving eye opening beforehand.
・Suitable for those with thick eyelids or sagging skin:The skin, orbicularis oculi muscle, ROOF fat, orbital fat, and other internal structures can be directly observed while excising unnecessary portions.
△Disadvantages:
・Longer downtimeBy the time of suture removal one week later, the swelling will have subsided to some extent. The final double eyelid width stabilizes in about one month and is fully established by three months.
This case underwent a full incision + droopy eye correction (glamorous line incision) + dark circle treatment (fat removal) simultaneously. As you can see, even one week post-op, the swelling is remarkably minimal.

Surgeon: Ryutaro Natsume (Immediately after surgery → 1 week post-op at suture removal)
・Cannot restore the original eyelidWe do offer procedures to narrow double eyelids or create single eyelids. We also perform revision surgery for procedures done at other clinics, so please consult with us.
