Treatment Methods Based on the Type of Skin Spot
As an Assistant Professor, Associate Ward Director, and Chief of Dermatopathology at the university hospital dermatology department, I deepened my expertise through academic presentations and paper publications both domestically and internationally. I have handled a wide range of treatments, from birthmark therapy in children to adult pigmentation concerns, utilizing various lasers and injection therapies, while also contributing to community healthcare. Through encountering numerous cases, I strongly felt the potential and necessity of aesthetic medicine, resolving to pursue further expertise. Building upon my solid dermatological knowledge and meticulous diagnostic skills, I value treatments that gently support each individual's skin while bringing out its inherent beauty.
We would like to propose treatments tailored to the specific type of skin spot.

I am Dr. Aya Hayama, a board-certified dermatologist.
Types of Skin Spots
What you might think are spots actually come in different types.
So-called age and photoagingSenile lentigineshormonal balance or stimulationliver spotsappearing from a young ageFreckles (ephelides)a bruise that appears laterADM (Acquired Bilateral Nevus of Ota, Acquired Dermal Melanocytosis), such as eczema or after laser treatment, which occurs after inflammationhyperpigmentationIf it's something you've had since childhoodFlat nevusIt can also be a type of birthmark.
Treatment Methods for Skin Spots
Liver spots can sometimes worsen with laser stimulation, and since the treatment approach for pigmentation also differs, it is crucial to begin with a proper diagnosis. However, since these conditions often coexist, we believe it is equally important to proceed with treatment selection by discussing downtime and the treatment progression with the patient.
Spots and freckles
For those with strong coloration and isolated features,PicoSure PicoSpotis recommended.
However, as part of the downtime, some scab formation may occur, and pigmentation may appear, peaking around one month after the procedure. Therefore, post-treatment care is also important.
For fine spots and faint frecklesIPL Treatment with LumeccaThis is recommended, but it may also cause some scabbing.
Since pigmentation is not as significant as with PicoSure, there is no need for tape application or similar measures.
For those who wish to undergo treatment without experiencing downtime, picotoningYou might also try gradually thinning it out.

ADM, flat nevus
ADM has melanocytes in the dermis,Several treatments at Pico Spotwill be required.
Flat birthmarks can be removed in about one-third of cases, but they do not always disappear. Therefore, we would like to discuss with you how far we should proceed with treatment.
liver spots
As for melasma treatment, fundamentallyTranexamic acid 500-750 mg/day, combined with vitamin CIt is said that 80% improve within three months.
Tranexamic acid is an anti-plasmin agent with mild coagulation effects. Therefore, individuals with hyperlipidemia cannot take it while on oral contraceptives due to the risk of thrombosis. That said, reports of thrombosis are extremely rare, so personally, I think it's acceptable to take it for a short period.
For those who cannot take oral medicationIntroduction of Tranexamic Acid via Mesona J and CosmeticsUsing it is also a good idea. I also think using injections or Target Cool is a good idea.
Also, be careful not to worsen melasma!!
When performing skincare, avoid excessive rubbing with cleansing products, vigorous patting, and be mindful that hormonal imbalances can also worsen the condition.
Post-inflammatory hyperpigmentation
Post-inflammatory hyperpigmentation often resolves naturally, but the duration varies from three months to about a year depending on the location and severity of the inflammation.Cosmetics containing tranexamic acid and hydroquinoneWell,Introduction of Tranexamic Acid in Mesona J, PiーRing, especially Bio PeelWe believe it is useful for pigmentation.picotoningやpotenzaTreatment for melasma is also effective.
Here's a personal musing from me, a dermatologist...
Many people have both sunspots and melasma. It seems a shame to avoid sunspot treatment out of fear of worsening melasma. Therefore, I think it's a good idea to first undergo melasma treatment for about two months as premedication, then monitor the progress while using treatments like Lumecca or PicoSpot.
