Fat grafting offers the appeal of "the reassurance of using your own fat" and "long-lasting results."
Last time we covered the basics, so this time I'll introduce some more specialized perspectives on fat processing methods and techniques for improving engraftment rates.

1. Fat Processing Methods and Types

Fat grafting does not involve injecting harvested fat directly; instead, it is processed according to the intended use before being injected.
There are several types of processing methods.

★Condensed Rich FaCRF

Approved by the U.S. FDA (equivalent to Japan's Ministry of Health, Labour and Welfare)The approved LIPOKIT and its accessories centrifuge harvested fat without exposing it to air, removing impurities such as dead or aged cells and cellular debris that cause fat necrosis, lumps, and calcification.

This fresh, concentrated fat from which impurities have been removed is called Condensed Rich Fat (CRF).
It boasts a high engraftment rate and is considered the most reliable option for volume restoration.

Microfibers sized to match concentrated fat

They can be classified into nano and nano-fat.
We often use larger sizes than micro or nano to add volume.

・Microfat
Compared to nanofat, the fat is coarser and larger in size.Volume-boosting effectis high.
On the other hand, it can also be used on delicate areas such as under the eyes and around the mouth.

・Nanofat
Further refine fat cells to increase the number of adipose stem cells and growth factors.

Including those in a liquid state.
Rather than for volume replacement, it is used to add thickness to the skin,"Improve skin texture" "Reduce pigmentation"is used.
It is also effective for fine lines and dark circles under the eyes.

2. Factors Influencing Retention Rates

The challenge with fat grafting is "how much fat survives." While it's generally said to be 30-70%, the reasons for variation are as follows.

・Collection Method
Strong suction can damage fat cells and reduce engraftment rates, so it is important to harvest carefully at low pressure.

・Processing method
Removing impurities and blood while concentrating the fat increases the survival rate of fat cells.

・Injection Technology
Placing them all together can cause necrosis or lumps, so the basic rule is to place them in small portions.

・Condition of the injection site
Fat grafts take better in areas with good blood flow. The take rate decreases in scar tissue or areas with poor circulation.

3. Combining Fat Injection with Regenerative Medicine

In recent years, methods combining adipose-derived stem cells and PRP (platelet-rich plasma) have emerged to enhance graft survival rates and skin quality improvement effects.

・PRP + Fat Injection
PRP is a plasma component rich in platelets and growth factors that activate cells.
By creating PRP from your own blood drawn beforehand and mixing it with fat for injection, it helps the fat to firmly take hold.
Recommended for those who want to maximize the longevity of the finish and complete the job in one go whenever possible.
The growth factors contained in PRP help fat cells take root while also offering skin quality improvement effects.

・Under the eyes → Improve shadows and dark circles with nanofat or microfat

・Cheeks and temples →Using larger fat particles to replenish volume

・Framed → From a flat forehead to a rounded, feminine contour
