前回の記事では、ハムラ法が「ふくらみ＋凹み」というクマの本質にアプローチできる最も合理的な治療であることを解説しました。

今回は、ハムラ法のメリット・デメリット・そしてその解決策をしっかり整理してお伝えします。

どんな治療にも長所と短所がありますが、ハムラ法も例外ではありません。

ただし、弱点は明確で、適切に説明して理解していただければ、十分にコントロールできるものばかりです。

■ メリット①：再発しにくい構造に変えられる

ハムラ法が脱脂と大きく異なる点は、眼窩隔膜（隔膜）を骨膜に固定してタイトニングする、という構造的変化を加えることです。

As a result,脂肪が前方に飛び出しにくくなり将来的な再発予防ができ、術後の状態が長期的に安定します。

脱脂は「脂肪を減らすだけ」なので再発しやすいと言われますが、ハムラ法は原因に対して構造的な変化を加えるため、根本治療に近いのが特徴です。

■ メリット②：表情がとても自然

脂肪注入では、眼輪筋内や眼輪筋上に脂肪が入っていると、笑った時にモコッと膨らむ、という現象が出ることがあります。

ハムラ法は、移動する脂肪が骨の直上というかなり深い位置に再配置されるため、表情を変えても不自然な膨らみが生じず、極めて自然な仕上がりとなります。

■ デメリット①：mid-cheek の深い凹みには届かない

ハムラ法で移動させられる眼窩脂肪には、到達距離の限界があります。

再配置できる範囲は、tear trough の溝から約 8〜10mm 程度is.

そのため、ゴルゴラインの下部であるmid-cheek grooveの尾側の凹みには脂肪が届かないため、ハムラ法だけでは改善できないことがあります。

これは“できない”というより、眼窩脂肪の解剖学的な位置から当然の制約です。

ただし、この範囲の凹みが問題になる場合は二期的に脂肪注入を追加することで十分に補えます。

ハムラ法で上部の構造を整えたうえで、必要な部分にだけ足していくというイメージです。

■ デメリット②：皮膚の薄さが原因の“色味”には限界がある

ハムラ法は凹凸による影に効果的な治療ですが、皮膚が薄くて眼窩脂肪の色が透けて見えているタイプでは、色味が完全には取れない場合があります。

これは構造ではなく皮膚の厚みの問題であり、ハムラ法だけでは限界がある部分です。

この場合に有効なのがナノファット注入で、目的はただ一つです。

皮膚の厚みを補うことによって透けて見える色味を軽減する、という厚み補正です。

凹凸（影）をハムラ法で整え、皮膚の薄さによる色味をナノファットで補うことで、より自然で明るい目元に仕上げることができます。

■ Summary

ハムラ法には「改善できない範囲」や「色味の限界」はありますが、それらはすべて明確な解決策が存在するコントロール可能な弱点です。

• mid-cheek の下方 → 脂肪注入で補完

• 皮膚の薄さによる色味 → ナノファットで厚み補正

一方で、

• 再発しにくい構造がつくれる

• 表情が極めて自然

• 脱脂だけでは実現できない“影の根本改善”ができる

というメリットの大きさは、ほかの治療ではなかなか得られません。

構造・持続性・自然さのバランスが非常に良く、クマ治療の中でももっとも完成度が高い手術のひとつと言えます。
