「目を大きく見せたい」と思った時に、上方向ではなく横方向に広げるという視点はとても大事です。

横方向に目を広げる手術の代表がcraniotomyとincision at the outer corner of the eyeis.

今回は外側方向の手術、目尻切開についての記事です。

実は目尻切開には術式がいくつかあり、その代表が単純切開 / W法 / 目尻靭帯移動 is.

どちらも“外側方向”の拡大ですが、変化や特徴が異なってきます。

この記事では、単純切開 / W法 / 目尻靭帯移動の3つを、メリット・デメリットを含めながら分かりやすく解説します。

■ 単純切開

目尻の皮膚を横方向に切り、白目の見える範囲を延長する方法。

● メリット

• 手術が比較的シンプルでダウンタイムが短め

• ほんのり外側へ広がる

• 強い変化を必要としない方に向いている

● デメリット

• 後戻りしやすい

• 目尻が丸く見えるリスク（※ グレイラインに切開を入れるため）

• 解剖的限界があり、大きな変化は望みにくい

※ グレイラインとは、まつ毛のすぐ上にある、まぶたの縁を形づくる細いラインのこと。皮膚と粘膜の境目で、まぶたの“シャープさ”を作る部分です。この連続性が途切れると、目尻がやや丸く見えやすくなります。

特に、グラマラスラインと組み合わせると、丸みが出る傾向にあります。

■ W法（W形成）

単純切開よりも後戻りしにくく、広がりを少し維持しやすい方法。

皮膚の切開ラインをW状にデザインすることで、拘縮（縮む力）を分散させるのが特徴です。

● メリット

• 単純切開より後戻りしにくい

• わずかにしっかり目に外側へ広がる

• 仕上がりの安定性が高い

● デメリット

• それでも大きな変化は難しい

• 単純切開同様、目尻が丸く見えるリスク

• デザインが複雑な分だけ腫れがやや強く出ることもある

■ 目尻靭帯移動（タレ目尻切開）

近年人気の高い方法。

外側の靭帯を一度外して、外側下方へ付け直すことで、目尻そのものの位置を変える手術。

● メリット

• 外側＋外側下方へ拡大できる（横＋縦の複合変化）

• 後戻りしにくい

• 目尻のシャープさが保たれる

→ “丸くならない”のが大きな利点

• つり目の方はつり目感がなくなり優しい印象になる

● デメリット

• ダウンタイムは目尻切開よりやや長め

• タウンタイムとして結膜浮腫が出るリスク

• 技術差が大きいため、医師の経験が重要

■ どれを選ぶべき？

• 自然でほんのり横に広げたい

　→ 単純切開

• 少ししっかり効果を出したいが自然が良い

　→ W法

• 横＋下方向に大きく見せたい／つり目を改善したい／後戻りを避けたい

　→ 目尻靭帯移動

特に、「丸くなるのが嫌」「シャープな目尻を残したい」方には

目尻靭帯移動が選択肢の候補will become.

■ Summary

目尻切開は“横方向の拡大”に特化した手術ですが、

仕上がりの印象は「どの方法を選ぶか」で大きく変わります。

• 単純切開：自然・軽い拡大・丸くなるリスク

• W法：少し強め・後戻りしにくい・丸くなるリスク

• 目尻靭帯移動：横＋下方向・後戻りしにくい・丸くなりにくい

横方向の拡大だけでも、技法によってこれだけ違いがあります。
