前回までの記事では、横方向に広げる目尻切開、そして下方向へカーブを下げるグラマラスライン形成について解説しました。

今回はその両方の良いとこ取りともいえる施術、「タレ目尻切開（目尻靭帯移動術）」について詳しく解説します。

最近SNSでもよく耳にするようになった術式ですが、「目尻切開と何が違うの？」「つり目改善に本当に効くの？」というご質問を多くいただきます。

実際は、目尻切開の発展版のような手術で、つり目が気になる方には最も効果的なアプローチといえます。

■ どんな手術？〜外眥靭帯を“移動させる”という新しい発想〜

下まぶたの外側には、外眥靭帯（外眼角靭帯）という強い靭帯があります。

これは、眼窩外側壁（頬骨）の内側に付着しており、

• 付着部が高い → つり目

• 付着部が低い → たれ目

という見た目の違いをつくっています。

つまり──

“靭帯の付着位置を変えれば、目の形そのものが変わる”

これがタレ目尻切開の本質です。

実際の手術では、① 一度靭帯を骨から外す、② 少し下方の骨へ再固定（縫着）する、という操作を行います。

これが目尻“靭帯移動“という名前の由来です。

靭帯を骨に固定するため、後戻りが非常に少ないのが大きなメリットです。

■ どんな変化が起こる？目尻切開との違い

タレ目尻切開では、目尻の溝に沿って切開を行い、外側下方へ白目が広がります。

• 横方向の変化（目尻切開）

• 下方向の変化（グラマラスライン）

この“両方の効果”を同時に得ることができます。

Also,グラマラスラインと併用することで、外側下方に最大限白目を広げることが可能になり、つり目の改善効果はさらに高まります。

■ 後戻りはどれくらい？ “1割の過矯正”がカギ

タレ目尻切開は後戻りが少ないと言われる施術ですが、経験上1〜2割程度は戻ります。

そのため私は、

• 靭帯移動：1割程度の過矯正

を行い、3ヶ月後に“ちょうど良い位置”になるよう調整します。

この見極めは経験値が非常に重要で、術者のセンスが出るポイントでもあります。

■ ダウンタイムの注意点：後戻りと結膜浮腫

ダウンタイムでよく見られるポイントは2つ。

① 後戻りの予測と過矯正

前述したように、術後1ヶ月は「少し下がりすぎ…？」と感じる方が多いですが、そこからまた戻るため、3ヶ月後に“本当の完成形”が出る施術is.

② 結膜浮腫

まぶたの内側がぷくっと腫れる現象。

通常は2〜3週間で改善しますが、抜糸時に穿刺・排液を行うことで早く落ち着きます。
