Hello!
Designing natural beauty with trust.
I am Masataka Fujihashi, a board-certified plastic surgeon.

In previous articles,Horizontal Eyelid Surgery, andForming a glamorous line that curves downwardWe have explained about.
This time, we'll explain in detail the procedure that combines the best of both worlds: the droopy-eye corner incision (lateral canthoplasty).
This procedure has become increasingly common on social media lately, but we often get questions like, "How is it different from outer canthoplasty?" and "Does it really work for improving upturned eyes?"
In reality,Advanced version of lateral canthoplastysuch as surgery,The most effective approach for those concerned about upturned eyesIt can be said.
⸻
■ What Kind of Surgery? — A New Approach: “Relocating” the Lateral Canthal Tendon
On the outer edge of the lower eyelid,Ligamentum palpebrae laterale (lateral canthal ligament)There is a strong ligament.

Source: Graphics Face Clinical Atlas of Anatomy (Quintessence Publishing Co., Ltd., 2018), p.197, Figure 2.24, reproduced with modifications.
This attaches to the inner surface of the lateral wall of the orbit (zygomatic bone),
• High attachment point → Upward-slanted eyes
• Low attachment point → Droopy eyes
creates a visual difference.
In other words──
“Changing the attachment point of the ligament alters the shape of the eye itself.”
This is the essence of droopy-eye corner surgery.
In actual surgery,① Detach the ligament from the bone once,② Reattach (suture) to the bone slightly lower down,Perform the following operation.
This is the outer corner of the eye“Ligament Transfer“That is the origin of the name.
To secure the ligament to the bone,Very little backtrackingis a major advantage.
⸻
■ What changes occur? Differences from lateral canthoplasty
For droopy-eye corner surgery,An incision is made along the outer corner of the eye, allowing the white of the eye to extend outward and downward.
In other words,
• Lateral change (eyelid incision)
• Downward change (Glamorous Line)
You can achieve both effects at the same time.

Also,When used in combination with the Glamorous Line,It is possible to maximally widen the sclera outward and downward. This further enhances the effectiveness of improving upturned eyes.
⸻
■ How much relapse? “10% overcorrection” is the key
Droopy-eye corner surgery is said to have minimal regression, butBased on experienceAbout 10 to 20 percent will return.。
Therefore, I
• Ligament transfer: Approximately 10% overcorrection
andIn three months, it will be in the perfect position. We will make adjustments.
This judgment requires significant experience,The point where the practitioner's sense comes throughas well.

⸻
■ Precautions During Downtime: Regression and Conjunctival Edema
Two common points observed during downtime.
① Prediction of Relapse and Overcorrection
As mentioned earlier, many people feel that their breasts are "drooping a bit too much..." during the first month after surgery, but they will gradually return to their original position.A treatment whose “true final form” emerges after three monthsis.
② Conjunctival edema
The phenomenon where the inner side of the eyelid swells up.
Typically, improvement occurs within 2 to 3 weeks, but puncturing and draining the area during suture removal can help it settle more quickly.
⸻
■ Who is it for?
• I want to fundamentally improve my upturned eyes.
• I want to fully open the whites of my eyes outward and downward.
• I want to change to a gentler impression
• I want to choose a treatment that's less likely to recur
• Just having an outer corner eyelid surgery wasn't enough.
⸻
■ In actual counseling sessions...
For those who came hoping for glamour, I will alwaysIntroducing Droopy Eye Corner SurgeryI will do so.
That is, in counselingWhen asked why they request the "glamorous line," over half of those who mention it actually say it's because they're concerned about their upturned eyes. That's why.
As a result,
• Lower Eyelid Blepharoplasty Only
• Droopy Eye Corner Surgery + Glamorous Combination
whichever you choose more than half is.
⸻
■ First, understand what type your eyes are.
Is it because of the upturned eyes, or because of the curve of the lower eyelid?
This significantly changes the optimal treatment approach.
Please allow me to carefully examine the condition of your eyelids.
We propose the best approach for you based on medical evidence and experience.
