I am Masataka Fujihashi, a board-certified plastic surgeon.

タレ目形成として有名な「グラマラスライン形成」と、最近注目されている「タレ目尻切開(目尻靭帯移動)」。

どちらも“優しい目元”をつくる施術ですが、得意とする方向性も、変化の強さも、後戻りのしやすさもまったく異なります。

この記事では、それぞれの特徴を比較しながら、「自分にはどちらが向いているのか」を分かりやすく解説します。

■ タレ目尻切開とグラマラスの“役割の違い”

まずは、この2つをシンプルに整理すると理解が早いです。

• Droopy Eye Corner Surgery　→ 目尻の位置そのものを下げる（＋外側に広がる）

• グラマラスライン　→ 下まぶたのカーブを下げる（目尻の位置はそのまま）

カウンセリングで「グラマラスラインを希望」とおっしゃる方に理由を伺うと、実は“つり目が気になるから”という方が半数以上いらっしゃる印象is.

しかし、真のつり目改善に最も効果的なのは、目尻そのものの高さを変える“靭帯移動”です。

■ 後戻りはどれくらい？

前回までの記事でいずれの術式も後戻りするというお話をしましたが、再度比較して解説します。

Droopy Eye Corner Surgeryは後戻りが少ないと言われる施術ですが、経験上1〜2割程度は戻ります。

そのため私は、靭帯移動であれば1割程度の過矯正を行い、3ヶ月後に“ちょうど良い位置”になるよう調整します。

それに対し、グラマラスラインは直後の変化量に対して5〜6割程度に後戻りしてしまいます。

そのため、グラマラスであれば3〜4割の過矯正が必要となるため、術直後は想像よりも下がり気味に見える方が多いです。

ただし、この“下がり気味”は適切な経過であり、3ヶ月ほどで自然に落ち着きます。

目尻靭帯移動とグラマラスラインを併用した場合の実際の経過としては、術後1ヶ月の段階で「少し下がりすぎている」と感じた方ほど、術後3ヶ月には「ちょうど良くなった」とおっしゃることがほとんどです。

一方で、術後1ヶ月で「ちょうど良い」と答えた方は、術後3ヶ月で「少し物足りないけれど、許容範囲です。」と感じるケースが多いという印象です。

この見極めは経験値が非常に重要で、術者のセンスが出るポイントでもあります。

■ どちらを選ぶべき？目的別の最適解

• つり目が気になる（優しい印象にしたい）　→ 靭帯移動

• 目の縦幅を大きく見せたい　→ グラマラス

• 外側下方に広げたい　→ 靭帯移動

• タレ目感が欲しい　→ グラマラス

• しっかり変化を出したい　→ 併用（靭帯移動＋グラマラス）

• 後戻りが心配　→ 術者の腕次第

• 最大限目を大きくしたい　→ 併用（靭帯移動＋グラマラス）

実際のカウンセリングでも、グラマラス希望の方の半数以上が靭帯移動または併用を選びます。

■ 完成までの流れとダウンタイム

どちらも完成は術後3ヶ月。

靭帯移動は結膜浮腫がやや起こりやすく、グラマラスでは術直後の“過矯正”に驚かれる方もいます。

しかし、これらはすべて適切な経過で、最終的には自然に落ち着きます。
