I am Masataka Fujihashi, a board-certified plastic surgeon.

目頭切開とは、上まぶたの内側にある「蒙古ひだ」を切開・解除することで、目頭をすっきり見せる手術です。

蒙古ひだは日本人を含む東アジア人に多く、これが強いと目が離れて見える、二重が末広型になりやすい、目頭が丸く幼い印象になる、といった特徴につながります。

目頭切開を行うことで、目の横幅が広がる、離れ目が改善する、平行型二重が作りやすくなる、大人っぽい洗練された印象になる、などの効果が得られます。

一方で「自然で傷が目立たない仕上がり」にするためには、蒙古ひだの構造を理解した精密なデザインと、丁寧な縫合が欠かせません。

■ Z法（Z形成）｜最もスタンダードで変化を出しやすい術式

Z法は古くから行われる代表的な術式で、切開線をZ字型にデザインします。

蒙古ひだのツッパリをしっかり解除でき、変化量を出しやすい方法です。

● メリット

• 蒙古ひだを確実に解除できる

• 平行型二重と相性が良い

• シャープでツンとした目頭を作りやすい

● デメリット

• 正面に傷が出るため早期は赤みが目立ちやすい

• 傷跡が残りやすい体質の人には不向き

• デザインや縫合で自然さに差が出る

Z法には大きく2つのバリエーションがあります。

① Z形成

蒙古ひだの稜線に切開を一致させることで凹面化し、ツッパリをしっかり解除できます。

ただし蒙古ひだが大きい場合、傷が斜めに広がることがあり、デザインに工夫が必要です。

使用するケース：

• シャープな目頭にしたい

• 蒙古ひだが大きくない

• しっかり変化を出したい

② ハーフZ形成

Z形成と似ていますが、Zの一辺を蒙古ひだの稜線に合わせずにデザインします。

蒙古ひだの方向（横、斜め下など）を微調整しやすいのが特徴です。

使用するケース：

• 蒙古ひだの方向を細かく調整したい

• 自然な変化を希望する

■ リドレープ法｜傷跡が圧倒的に目立ちにくい最新術式

リドレープ法は、蒙古ひだを皮膚ごと剥離し、位置を再配置することで形を整える方法です。

Z字に切開しないため、傷がまぶたの縁に沿って隠れ、非常に自然に仕上がるのが特徴です。

● メリット

• 傷跡が圧倒的に目立ちにくい

• 直後から比較的自然

• 他院修正などの微調整に向いている

● デメリット

• ツンとした鋭い目頭は作りづらく、丸みが出ることが多い

• 変化量は控えめ

• 高度な技術が必要で術者による差が大きい

■ 術式の選び方｜優劣ではなく“相性”が大切

Z法・ハーフZ形成・リドレープ法にはそれぞれ特徴があります。

どれが優れているかではなく、あなたの目元に最適なのはどれかが最も大切です。

術式選択の目安

• 平行二重・しっかり変化 → Z法 or ハーフZ法

• 傷を最小限に → リドレープ法

• 自然な変化・微調整 → リドレープ法 or ハーフZ法

• 左右差が強い → デザイン次第でいずれの術式も可能
