I am Masataka Fujihashi, a board-certified plastic surgeon.
鼻整形は、「鼻のどこを」「どう変えたいか」によって、必要な施術が大きく異なります。
むやみに高さだけ足しても、似合う顔立ちになるとは限りません。
今回は、鼻の構造に基づいて大きく6つに分類し、それぞれの代表的な施術と考え方を分かりやすく整理します。
まずは、ご自身の“気になるポイント”と照らし合わせてみてください。

① 鼻筋の高さを出す（鼻根〜鼻筋ラインの改善）
ヒアルロン酸注入（非手術）
手軽に形の試しができる方法。鼻筋の輪郭をくっきりさせます。
※鼻先への直接注入は血管閉塞のリスクが高く、基本的に行いません。
高さを出したい場合は膜性鼻中隔に注入します。
隆鼻術（手術）
・プロテーゼ：しっかりと高さが出て形が安定
・肋軟骨移植：自家組織のため長期的な感染リスクが少ない
粉砕（破砕）軟骨を用いて自然なラインの調整も可能です。

② 鼻先の形を整える（丸さ→シャープに／高さを出す）
鼻先の印象は 大鼻翼軟骨の形態が大きく関わります。
鼻尖形成術
軟骨を縫縮して、丸い鼻先をすっきりした形へ。
必要に応じてonlay graftやshield graftなど、軟骨移植も組み合わせます。
Septoplasty
鼻先をさらに高くしたい場合の土台づくり。
また、鼻先の方向の調整の自由度も格段に上がります。
使用軟骨：肋軟骨／鼻中隔軟骨／耳介軟骨
元々エンベロープ（皮膚・軟部組織）に余裕がない方は、鼻中隔延長がないと十分な変化が出ないことがあります。

③ ワシ鼻・段差を改善（鼻筋を滑らかなラインへ）
Hump removal
鼻筋の出っ張り（骨・軟骨の段差）を削って横顔のラインを整えます。
隆鼻術との併用も多い施術です。

④ 横幅や歪みを整える（正面の印象を大きく変える）
Nasal bone osteotomy with narrowing
鼻筋が広い・曲がって見える…といった悩みを改善。
左右差が強い方にも有効です。

⑤ 小鼻・鼻孔の形を整える（正面からの印象改善）
鼻翼縮小術
・内側法：鼻の穴が大きいのを改善
・外側法：小鼻の広がりを改善
鼻孔縁挙上／下降
・挙上：小鼻の厚み改善
・下降：鼻の穴が正面から見えるのを改善
どの部位が小鼻が大きく見える原因かを見極めることが大切です。
穴？広がり？厚み？で手術内容は変わります。

⑥ 中顔面の立体感を補う（中顔面の立体感を整える）
貴族手術
鼻の付け根が凹んでいる方に適応。
中顔面が立体的になり、ほうれい線が浅く見える効果も期待できます。

■ 手術選びの正しい考え方
鼻整形で最も大切なのは「原因を見極めること」です。
さらに、鼻先と小鼻が両方気になる場合は必ず、鼻先 → 小鼻の順でアプローチI will do so.
鼻先が整うと、小鼻の広がりが落ち着く場合が多いからです。
