鼻尖形成（びせんけいせい）とは、鼻先（鼻尖）の形を整えて、丸み・広がり・左右差を改善する手術です。

鼻先をつくっている大鼻翼軟骨の形態を調整することで、正面からも横顔からも洗練された印象に仕上げます。

団子鼻が気になる方に選ばれることが多い施術です。

■ 鼻尖形成で変えられるもの

鼻先の、

丸さ（団子鼻）/ 横幅の広がり / 輪郭の曖昧さ / 左右差 / わずかな高さ

これらを自然に改善できます。

ただし高さをしっかり出したい場合は、別の施術を組み合わせる必要があります。

■ 基本の手術ステップ

鼻尖形成は大きく分けて2つのアプローチがあります。

① 大鼻翼軟骨を寄せて形を締める

軟骨を中央に縫い寄せ、丸い鼻先を細くします。

皮下脂肪が厚い方は、同時に脂肪・軟部組織を少し取り除き、丸みの原因から解消します。

② 軟骨移植を追加して、形と強度を補う

軟骨が小さい方・柔らかい方は、寄せるだけでは高さや輪郭が出にくいため、耳介軟骨などを移植します。

・鼻先の輪郭を整える：onlay graft / shield graft

・鼻柱側から支える：columellar strut（コルメラストラット）

日本人は鼻翼軟骨が柔らかいことが多く、移植を併用することで後戻りを防ぎ、仕上がりが安定します。

■ 鼻中隔延長との違い

・鼻尖形成

→ 鼻先そのものを「整える」手術

→ 輪郭・横幅・わずかな高さ改善

・鼻中隔延長

→ 鼻先の「土台を伸ばす」手術

→ 高さ・向き・安定性を強化

しっかり高さを出したい、鼻先を方向も変えたい場合は鼻中隔延長術の適応となります。

※ 鼻整形をする方は基本的に全ての症例で鼻尖形成は行う形となります。

■ ダウンタイム・経過

• 腫れ：1〜2週間

• 最終形：3〜12ヶ月かけて安定

• テーピング固定あり

鼻先は皮膚が厚く動きのある部位のため、仕上がりには時間をかけて形が馴染んでいきます。

鼻尖形成は

✔ 団子鼻をスッキリさせる

✔ 鼻先の横幅と輪郭を整える

✔ 正面も横顔もスマートに

することができる手術です。

一方で、高さや向きの大きな変化は土台手術（鼻中隔延長）が必要です。

鼻先の皮膚の厚さ、軟骨の強さ、まわりの組織の余裕など、それぞれを診断したうえで、最適な術式を選択します。

理想の「鼻先」を叶えるために、まずは原因をしっかり見極めることが最初の一歩です。
