I am Masataka Fujihashi, a board-certified plastic surgeon.

 

When viewed from the side, the bridge of the nose appears to protrude in the middle.

The uneven surface creates a rough and rugged impression.

This type of nose shape is commonly called an "eagle nose," and the protrusion causing it is referred to as theHumpIt is called.

Because the nose is positioned at the center of the face, even slight irregularities can significantly alter one's overall impression.

Nasal hump reduction surgery is a procedure to smooth out the bump on the bridge of the nose and create a more seamless contour.

 

 

■ What is a hump nose?

An aquiline nose is a general term for a nose that appears prominent and arched along the central bridge when viewed from the side.

In cases of pronounced protrusion, it may be described as a "hooked nose" or "witch's nose," unintentionally creating a strong impression.

In particular, a condition where the bridge of the nose has a step-like indentation is called a "hump."

The hump is formed at the boundary between the hard tissue of the nasal bone in the upper half of the nose and the soft tissues of the nasal septal cartilage and lateral nasal cartilage in the lower half.

Therefore, simply shaving it down will not create a natural line; adjustments must be made considering both bone and cartilage.

 

 

■ What is a Hump Nose Correction (Hump Reduction)?

Hump reduction surgery involves removing bone and cartilage from the protruding area along the nasal bridge to create a smoother nasal profile.

Properly treating the hump will straighten the bridge of the nose, giving the entire nose a more refined appearance.

The characteristic feature is that the state where only the nose appeared emphasized is improved, resulting in a more balanced appearance of the entire face.

The goal is not simply to lower the height, but to eliminate the step.

■ Surgical Technique for Hump Reduction

The nasal bone portion, which constitutes the upper half of the hump, is removed using tools such as chisels.

The lower cartilage portion is carefully adjusted with a scalpel to ensure a smooth transition between bone and cartilage.

Furthermore, by adjusting not only vertical but also horizontal irregularities, we create a natural nasal bridge line.

In cases where only hump reduction is performed, the closed approach—operating from inside the nostrils—is often sufficient.

■ The Concept of the Closed Method and the Open Method

Rhinoplasty for a hooked nose employs either the closed or open technique depending on the specific case.

For hump shaving alone, when the amount to be shaved is small, the closed method is used.

On the other hand, when combining rhinoplasty with procedures to raise the nasal bridge, performing other nasal surgeries simultaneously, or when the hump is large and requires significant removal, the open technique is generally selected.

Prioritizing the accuracy and safety of the finished product, it is important to determine which method is appropriate.

 

 

■ Hump Removal and Spreader Graft

In cases of a large hump, structural defects may occur in the central nasal bridge after excision.

This area is also related to the nasal air passage, and failure to provide adequate reinforcement may cause nasal congestion.

Therefore, a spreader graft using nasal septal cartilage is used as needed to stabilize both the nasal bridge line and function.

In such cases as well, the open approach, which allows for a clear field of view, is suitable.

 

 

■ Summary

Nasal hump reduction is not a simple procedure that merely shaves down the protruding nasal bridge.

This surgery creates a natural nasal bridge line by understanding the structure of bone and cartilage and smoothly connecting any unevenness.

The choice between the closed method and the open method is determined on a case-by-case basis, with the highest priority given to the final result and safety.

When concerned about the appearance of the nasal bridge, it is important to consider treatment while keeping in mind the overall balance of the nose.
