鼻中隔延長は、鼻先の向き・高さ・安定性を立体的にコントロールするための土台形成術です。
しかし、この手術で最も重要でありながら見落とされやすいのが、延長の支えとなる鼻中隔軟骨そのものの状態です。
土台が歪んだまま延長を行えば、どれほど丁寧にデザインしても鼻先は傾きやすく、左右差や後戻りの原因になります。
だからこそ、鼻中隔延長と鼻中隔湾曲は切り離して考えることはできません。

■ 鼻中隔がまっすぐであることの重要性
鼻中隔延長では、鼻中隔軟骨が“柱”となり鼻先を支えます。
そのため、この軟骨がストレートであれば、設計通りに真っ直ぐ延長することができます。
一方、湾曲した鼻中隔をそのまま使うと、延長軟骨も同じ方向へ引かれ、鼻先が傾くリスクが高まります。
軽度であれば配置の工夫で補正できますが、高度な湾曲では事前の矯正が不可欠is.

■ 鼻中隔湾曲症とは
鼻中隔湾曲症とは、左右の鼻腔を隔てる鼻中隔（軟骨・篩骨・鋤骨）が曲がっている状態を指します。
日本人の約9割に何らかの湾曲があるとされ、決して珍しいものではありません。
しかし、湾曲が強くなると鼻づまり、口呼吸、嗅覚低下、頭痛、いびきなどを引き起こし、日常生活に支障をきたします。
症状改善のみを目的とした矯正手術は保険適応However,美容手術と同時に行う場合は自費診療will be.

■ 鼻中隔湾曲があると起こる整容面と機能の問題
湾曲した鼻中隔は、鼻筋の歪み（斜鼻）や鼻先のズレの原因になります。
骨性斜鼻と軟骨性斜鼻が合併しているケースも多く、両方を同時に整えなければ左右差は改善しません。
さらに一側の鼻腔が狭くなると、反対側では下鼻甲介が肥大し、両側鼻閉を生じることもあります。

CTで湾曲の位置と程度を正確に評価し、必要に応じて鼻中隔軟骨を切離・再固定します。
温存した軟骨に浅い切開を加えて直線化し、補強のために軟骨を移植することで再発しにくい構造を作ります。
この過程で得られる良質な軟骨は、延長術や鼻尖形成の自家材料としても重要です。

■ 期待できる効果
鼻呼吸の改善、慢性的な鼻づまりやいびきの軽減といった機能的効果に加え、鼻筋・鼻先の歪みが整い、顔全体のバランスが改善します。

■ 鼻中隔湾曲矯正の合併症・リスク
感染、左右差、曲がり、後戻り、鼻中隔穿孔(穴が開く)、鼻閉の悪化、鞍鼻、術後出血などが起こる可能性があります。
鼻中隔や粘膜は血流が豊富なため、術後1週間程度は出血を伴うことがあり、稀に止血処置が必要になる場合もあります。
そのほか、内出血、腫脹、疼痛、感覚障害、血腫、糸の露出などが生じることがあります。

■ Summary
鼻中隔延長を成功させるためには、延長の前に鼻中隔の状態を正確に評価することが不可欠です。
湾曲を見逃したまま手術を行うと、鼻先の傾きや左右差、後戻りにつながる可能性があります。
必要に応じて湾曲矯正を併用することで、構造の安定性と仕上がりの自然さが高まります。
