We will mail the "Personal Information Disclosure Request Form" to your address. Please complete the required information and return it by mail along with your identification documents.

If the agent is a legal representative such as a custodial parent, you may submit documentation verifying your relationship with the principal instead of a power of attorney.
④ Fees and Collection Methods
For each billing cycle, please transfer the tax-inclusive handling fee of ¥1,100 to the designated bank account.
Please note that all expenses related to requests for disclosure, such as printing and copying fees, costs for obtaining identification documents, postage, and other necessary expenses, shall be borne by the individual making the request. Additionally, please be advised that if the required fee is insufficient, we will be unable to provide disclosure or notification of the purpose of use.
⑤ Method of Responding to Requests for Disclosure, etc.
We will respond in writing to the address provided by the claimant. If you have any questions, complaints, or concerns, please contact the Complaints and Consultation Desk listed in Section 8.① above.
⑥ Purpose of Use for Personal Information Obtained Through Disclosure Requests, etc.
Personal information obtained in response to disclosure requests shall be handled only to the extent necessary for processing such requests. Submitted documents shall be retained for one year from the date the response to the disclosure request is completed, after which they shall be destroyed.
⑦ Regarding Reasons for Non-Disclosure
Disclosure will be withheld in the following cases. If disclosure is withheld, notification will be provided stating the reason.
- ・ If we cannot verify that the request is from the individual themselves, such as when the address on the application form, the address on the identification documents, and our registered address do not match.
- ・ When applying through an agent, if the agent's authority cannot be verified

- ・ When the subject of the disclosure request does not fall under the category of retained personal data or third-party provision records as defined in Article 16 of the Act
