Removal of dark circles and sagging under the eyes (transconjunctival blepharoplasty)

For mild puffiness, fat removal alone can effectively reduce dark circles. However, for those with significant puffiness, dark circle reduction through fat removal alone may lead to increased skin laxity or more wrinkles. For individuals with mid-face hollows or a diagonal hollow extending from the inner corner of the eye to the center of the cheek (the so-called "Gorgon line"), combining fat grafting is also recommended. In cases of pronounced puffiness or skin laxity, lower eyelid skin excision may be necessary.

Dark circles and under-eye puffiness may recur. Particularly with liposuction alone, some individuals experience recurrence as early as 2-3 years after treatment, finding themselves in a situation where "dark circles remain despite having had them removed." Depending on the condition, dark circle removal may require multiple surgeries. The standard procedure involves harvesting fat from the thighs. For slender individuals with insufficient fat reserves, or those who have undergone liposuction on the thighs within the past six months, fat is harvested from the abdomen. During the procedure, the fat harvested from the patient is purified and then injected.

You may experience some stinging or a mild, muscle-ache-like heaviness for about 3 weeks. Additionally, the thigh where fat was harvested will temporarily feel firm, so please begin massaging the area 2 to 3 weeks after the under-eye fat removal surgery.

Individual results may vary, but for fat removal alone, recovery takes about one week. If fat grafting is also performed, recovery takes approximately 10 days to two weeks. Swelling typically peaks 3 to 4 days after surgery. Since swelling mainly occurs under the eyes, wearing a mask pulled down over the lower face helps minimize its appearance and reduces disruption to daily life. Some patients worry about how long the pain from under-eye fat removal will last; generally, the peak of discomfort lasts about 2 to 3 days.

While we often refer to dark circles simply as "dark circles," they actually come in three types: black (shadow) circles, purple circles, and brown circles. Unfortunately, bear-catching surgery won't eliminate wrinkles. Depending on the type of wrinkle, fat grafting may slightly reduce them, but it won't remove all wrinkles. Wrinkles that appear when you smile are caused by facial muscles, so Botox injections can reduce them.
