I am Masataka Fujihashi, a board-certified plastic surgeon.

鼻の印象は、顔全体のバランスを大きく左右します。

特にアジア人は、鼻根部（鼻の付け根）が低かったり、横から見たときに平面的に見えやすい骨格の方が多い傾向にあります。

そんな控えめな鼻筋を解消し、顔の立体感を高める治療として多く行われているのが鼻プロテーゼ（シリコンインプラント）による隆鼻術is.

プロテーゼとは、医療用シリコンで作られた人工軟骨のこと。

心臓の人工弁などにも使われている安全な素材で、鼻骨の上に挿入することで鼻筋を通し、シャープなラインを作ります。

ヒアルロン酸注入と違い、半永久的に効果が続く点が最大のメリットです。

■ プロテーゼの種類と現在の主流

プロテーゼには「I型」と「L型」が存在します。

過去にはL型も多く使われていましたが、現在L型はほとんど使用されません。

なぜなら、L型は鼻先までプロテーゼが入るため

・鼻先の皮膚が薄くなり破れて露出する

・強い圧がかかり感染リスクが上がる

といった長期的トラブルが起こりやすいからです。

現在はI型がスタンダード。

鼻先は人工物ではなく、自家軟骨（耳介軟骨など）で整えるのが安全で自然です。

鼻先にプロテーゼを頼る施術は、今は推奨されません。

■ 手術の重要ポイント

プロテーゼは必ず骨膜下に入れることが大切です。

誤って骨膜上に入ってしまうと…

• プロテーゼが動く

• 触るとグラつきが分かる

• 将来的にズレや変形が起こる

他院修正相談で最も多い失敗の原因がコレです。

経験豊富な医師であれば、丁寧な剥離と固定で安定性を確保できます。

■ 鼻先も高くしたい場合

鼻先にまでプロテーゼを無理に乗せると、前述したように皮膚が耐えられず飛び出しのリスクが上昇します。

鼻先を整える際は、鼻尖形成 / 鼻先軟骨移植 / 必要に応じて鼻中隔延長

これらと組み合わせることで、安全かつ美しく仕上がります。

■ ヒアルロン酸注入との違い

ヒアルロン酸は手軽でダウンタイムが少ない一方、吸収されるため継続が必要です。

また、繰り返すほどに横へ広がり太く見えるリスクも存在します。

その点、プロテーゼは一度で長期的な変化を得られる方法といえます。

どちらを選ぶかは、「期間」「形」「ライフスタイル」などを総合的に考えるのが理想です。

■ Summary

鼻筋をしっかり高くかつ、半永久的な変化を求める方には、プロテーゼによる隆鼻術は強力な選択肢です。

ただし、

• 鼻先は自家組織を基本に

• 骨膜下へ正確に挿入

• 他の施術とのバランス設計が重要

という点は必ず押さえておきたいところです。
