I am Masataka Fujihashi, a board-certified plastic surgeon.

 

When we think of contouring treatment, we tend to focus on the sharpness of the facial line, the volume of the gills, and the leanness of the sides of the mouth.

However, the morphology of the jaw is the “final piece” that actually determines the beauty of the contour.

No matter how small your face is, it will not look beautifully contoured without a well-shaped chin.

On the other hand, just by having a well-defined jaw, the overall impression of the face can often be refined at once.

 

 

Why the chin determines the completeness of the contour

The human face is evaluated not only in the frontal plane, but also in the "front-back three-dimensionality".

The well-known standard for profile isE line (line connecting the tip of the nose and the tip of the chin)is.

In an ideal situation

Upper lip slightly inside the line

Lower lip almost above the line

It is located at

 

A short chin makes the mouth appear protruding, while an overly protruding chin gives a squat appearance.

In other words, the chin is the “end point of the contour,” and if this point is out of alignment, the overall balance is lost.

Vertical proportions are also important for a beautiful lower face.

Lower nose to upper lip" : "Lower lip to chin tip" ≈ 1:2

This is the reason why liposuction and thread lifts alone are not completely solid because they are unadjusted.

 

 

Miter muscles affect balance.

A major cause of jaw morphology is overstrain of the miter muscle.

In other words, the muscles, not the skeleton, distort the contours.

In this type, chin hyaluronic acid alone is not enough to prepare the chin.

Differences in treatment methods

There are several stages of jaw formation.

In a broad sense, hyaluronic acid, prostheses, and osteotomies are all included in “odontoplasty,” but the term "odontoplasty" generally refers to osteotomies.

 

Firsthyaluronic acid injection

It is the easiest to use and excels in fine-tuning the forward/backward and downward directions.

It is the first choice for E-line improvement and is suitable for mildly receding jowls and contour balancing.

 

nextprosthesis insertion

Silicone implants allow for stable maintenance of shape and are indicated for moderately receding jaws.

It is suitable for cases that require semi-permanent volume and a certain amount of protrusion.

 

AndOsteogenesis (osteotomy)

By cutting, moving, and shaving the bone at the tip of the mandible, the length, anterior-posterior position, and left-right differences of the jaw are fundamentally improved.

Because the approach is made from within the oral cavity, no scarring is left on the skin, and skeletal-level problems such as squatting, retraction, and asymmetry can be corrected.

Osteotomies not only shorten, but can also move the bone fragments forward to bring the jaw forward.

Swelling will subside in 2-4 weeks, but complete stabilization may take several months.

Although there are risks inherent in surgery such as nerve palsy, infection, and hematoma, it is the most reliable method of contour improvement.

While there is a risk of bony resorption of the jawbone over the long term if an excessive amount of hyaluronic acid is injected or an unreasonably sized prosthesis is inserted, the risk is basically quite small with oroplasty.

 

In many cases, Japanese people have anterior-posterior protrusion and insufficient vertical length of the jaw, and treatment is usually in the direction of "supplementation" rather than "reduction".

Therefore, in many cases, hyaluronic acid injections or prostheses are sufficient to balance the volume, and osteotomies are not necessary in many cases.

On the other hand, if the jaw is too long or there is a left-right difference, shortening or adjustment through osteotomy is necessary.

 

 

Jaw is not the “final tweak”.

The chin is a very important element in achieving a beautiful contour.

If this area is ignored, no amount of other treatment will complete the process.

Contouring treatment is not simply about fat reduction, but must be aligned to a three-dimensional end point.

When the jaw is aligned, the balance of the entire face is naturally harmonized for the first time.
