小顔整形のカウンセリングでよく聞かれるのが、「頬の脂肪吸引とバッカルファット除去は何が違うのか？」という質問です。
どちらも顔のボリュームを減らす施術ですが、アプローチする脂肪の位置がまったく異なります。
この違いを理解せずに施術を選ぶと、理想の変化が出ないこともあります。
今回は、バッカルファット除去と頬の脂肪吸引の違い、そして実際にどの脂肪を治療すべきなのかについて解説します。

■ バッカルファットとは
バッカルファットとは、頬の深い位置に存在する脂肪（深部脂肪）is.
咬筋の前方、頬の内側付近に存在し、頬中央のボリュームを構成する脂肪のひとつです。
この脂肪が多い場合、頬の中央に厚みが出やすく、中顔面の丸みやボリューム感の原因となることがあります。

■ バッカルファットが多い人の特徴
バッカルファットのボリュームが多い方には、いくつか特徴があります。
・頬の内側をよく噛んでしまう
・口内炎ができやすい
・歯と歯の間に白い線（咬合線）がある
といった所見が見られることがあります。
これは頬の内側の粘膜が歯に当たりやすくなっているためです。
このような口腔内の変化は、バッカルファットの量を判断する際のひとつの参考になります。
また、奥歯を噛みしめたときに咬筋の前方に厚みがある場合も、バッカルファットによるボリュームであることがあります。

■ 頬の脂肪吸引との違い
頬の脂肪吸引は、皮膚のすぐ下にある皮下脂肪をカニューレで吸引する施術is.
フェイスラインや頬、あご下のボリュームを整える目的で行われます。
一方、バッカルファットは頬の深部脂肪であり、口の中からアプローチして摘出します。

という違いがあります。
脂肪の存在する層が異なるため、改善できる部位や変化の出方も異なります。

なお、いわゆる「口横ポニョ」と呼ばれる膨らみの多くはジョールファットby.
ジョールファットは口角下付近に存在する皮下脂肪で、
・口横のもたつき
・Marionette lines
・フェイスラインの重さ
の原因になることがあります。
一方、バッカルファットは頬中央〜中顔面のボリュームに関与する脂肪です。
そのため、フェイスラインのもたつきの原因とは必ずしも一致しません。
このように、顔には複数の脂肪層が存在しており、どの脂肪が原因かを見極めることが非常に重要です。

■ 治療は脂肪の層に合わせて選ぶ
顔には皮下脂肪と深部脂肪（バッカルファット）など複数の脂肪層が存在しており、ボリュームの原因となっている脂肪の層によって治療法を選択することが重要is.
一般的には、バッカルファット除去は頬コケのリスクと隣り合わせの施術であるため、まずは皮下脂肪にアプローチできる頬の脂肪吸引でボリュームをコントロールする方が安全なケースが多いと考えています。
皮下脂肪は顔の広い範囲に分布しているため、フェイスライン、頬、あご下などをバランスよく整えることが可能です。
On the other hand,丸顔で頬中央の厚みが強い方では、頬の奥の脂肪であるバッカルファットが原因である場合が多く、バッカルファット除去が非常に有効です。
However,バッカルファットは取り過ぎると時間経過で頬コケが目立つ可能性があるため、適応を慎重に見極めたうえで、必要な量だけを調整することが重要です。
このように、どちらの施術が優れているというものではなく、ボリュームのある部位と脂肪の層に応じて適切に使い分けることが、美しい仕上がりにつながります。

バッカルファット除去と頬の脂肪吸引は、どちらも小顔治療ですが、アプローチする脂肪の層が異なります。
頬の脂肪吸引は皮下脂肪、バッカルファット除去は頬の深部脂肪に対する治療です。
また、口横のもたつきの多くはジョールファットによるものであり、これも別の脂肪です。
顔の脂肪はそれぞれ役割を持っているため、すべて取れば良いわけではありません。
原因となる脂肪の層を正しく見極め、適切な量を調整することが自然で美しい輪郭を作るポイントになります。
