This patient presented with significant sagging, as well as irregular double eyelid lines resulting from multiple previous suture procedures and double eyelid surgery. We performed double eyelid surgery to create a single, defined crease while simultaneously removing excess skin. Although we did not perform ptosis surgery (such as levator advancement), the heaviness was eliminated and the eyes opened slightly better.
The primary approaches for removing upper eyelid sagging are either below the eyebrow (incision) or along the double eyelid line (double eyelid incision).
