Specialist in rhinoplasty. He is a board certified plastic surgeon by the Japanese Society of Plastic and Reconstructive Surgery. Based on the principle of “maximum effect with minimum treatment,” I propose the best surgical techniques and plans for my patients from various angles. He specializes in designs that beautifully accentuate the "individuality" of the patient while taking into account the golden ratio of the face. In particular, we create full-custom-made prostheses by combining CT and Vectra data to create full-scale blueprints, which are then precisely finished based on clay prototypes handmade from scratch on a skull model. The shape of the prosthesis can be reproduced with high precision as per the presented simulation, so that even the most detailed wishes can be met and stable results can be achieved.
Surgery to improve the irregularity of the base of the nasal wings and the base of the nasal bridge with less retrogression.
Although hyaluronic acid injections, condense rich face (CRF), and other procedures that fill in deep grooves are commonly used to approach laugh lines, it is difficult to see a change in deep lines caused by a sinking of the nasal wing base. The nasal wing base is the area at the base of the nose. A sunken nasal wing base can make the upper lip appear to protrude due to the difference in height. It is sometimes described as a "mouth sag" or "lip sag.
At BIANCA CLINIC, we focus on these structural causes and take an “adjust from the foundation” approach, such as aristoplasty and cat surgery.
Noble surgery lifts the base of the nose to soften the shadows of the laugh lines, while cat surgery reinforces the base of the nasal bridge to reduce the protruding mouth. Both are less likely to regress than hyaluronic acid or fat injections and can be expected to produce more long-lasting changes.
While the most common cause of mouth sores is the teeth and gums, they may actually be caused by a sunken nasal wing base or the shape of the base of the nasal column. It is important to find the appropriate consultation and treatment for your problem.

CHARACTERISTICS
What is Nasal Wing Base/Artist Surgery?
Nasal Wing Base/Artist Surgery is a procedure that pushes up the nasal wing base from the inside to increase its height and alleviate deep lines and protrusion around the mouth. The nasal wing base is the area at the base of the nose, and if it is sunken in, not only do the lines become more prominent, but the difference in height between the sunken area and the base of the nose can make the mouth appear more prominent. By adding a three-dimensional effect to the base of the nasal wings, the entire face is balanced, giving it an elegant and three-dimensional impression. The result is also called "aristocrat surgery" in Korea.
The most common method is to use a prosthesis (artificial cartilage). The scar will not be noticeable from the outside because the prosthesis is inserted from inside the mouth through an incision of a few millimeters at the base of the gum.
What is Nasal Wing Base/Artist Surgery (Fine Rib Cartilage Graft)?
In addition to inserting a prosthesis, aristoplasty surgery can also be performed by shredding and transplanting your own rib cartilage. The use of autologous tissue has the advantage of being biocompatible and easily blends with the tissue. Furthermore, compared to artificial materials, it is easier to achieve a natural finish and can be expected to have a stable effect over the long term.
However, because a small incision must be made in the chest to harvest the rib cartilage, this procedure is rarely performed alone and is often selected in cases where it is performed simultaneously with nasal septal lengthening.
Before the procedure, the presence of calcification is confirmed by CT, and rib cartilage in good condition is selected. The extraction is performed from a location that minimizes scarring, so that a natural appearance can be maintained even after the surgery.
What is cat surgery?
Cataract surgery is a procedure that raises the base of the nasal bridge (the depression between the nose and upper lip) and adjusts the nasolabial folds to an ideal angle. If the base of the nasal bridge is concave, the base of the nose appears concave in profile, and the upper lip and mouth appear forward, giving an unrefined impression. The patient's own cartilage is cut into small pieces and grafted into the nose.
The purpose of cat surgery is to bring the nasolabial angle closer to 90-95 degrees, which is considered ideal for Asians, and to improve the E-line. The name "cat surgery" is a sobriquet given by Korean doctors to the procedure because it makes the profile look cute like a cat's. Officially, it is called nasolabial angioplasty.
What is MRSA testing (nasal MRSA test)?
Bianca Clinic can also perform a MRSA test (nasal MRSA test) prior to performing nasal procedures.
The MRSA test is a test to check for bacteria present in the nasal cavity, particularly methicillin-resistant Staphylococcus aureus (MRSA). The test is performed simply by lightly swabbing the mucous membrane of the nasal cavity with a special cotton swab and is virtually painless.
Surgery is normally performed under aseptic conditions to prevent the entry of pathogenic microorganisms and contact infection, but the nose is structurally a difficult site to clean and manipulate. The interior of the nose is composed of cartilage, which is susceptible to infection and requires more careful hygiene.
The nasal cavity contains a large number of commensal bacteria, which basically do not cause infection due to the barrier function of the human body. However, there are a certain number of people who harbor risky bacteria such as MRSA, which can cause postoperative infection.
Because of the risk of postoperative infection, which can affect the finished product and functional aspects, MRSA testing in advance can help reduce the risk of infection.
Benefits
MERITElegant and refined profile line
The angle of the mouth (nasolabial angle) is adjusted from below the nose to form a natural and elegant E-line.
Reduce the appearance of lines and protruding mouth
Replacing the hollows at the base of the nose and upper part of the midsection will make the lines appear shallower and the protrusion of the mouth less prominent, thereby reducing the appearance of aging and thinning out.
Long-term changes can be expected.
Because prostheses and fine rib cartilage are used to firmly push up the base of the nasal wings and the base of the nasal column, the changes are longer lasting than with injection procedures.

RECOMMENDED POINTS
Offering patient-specific smile line treatment
Behind the deep and prominent lines are not only the sunken nasal wing bases, but also various other factors such as sagging skin due to aging, dryness, teeth, swelling, and so on. If sagging skin is the cause of the prominent lines, a "thread lift," "medical HIFU," "Volnuma," or "facelift" may be appropriate for the patient. At Bianca, our experienced doctors will carefully investigate the cause of your problem, and determine and recommend the most appropriate treatment, including aristosurgery.
The nose is an important part of the face that determines the impression of the face. We will listen to your wishes in detail to find a design that you will be satisfied with.
Combination treatment is an option for nasal concerns.
Bianca Clinic is staffed with highly experienced specialists in rhinoplasty, including plastic surgeons certified by the Japanese Society of Plastic Surgery and technical advisors. We can also offer combination or full custom treatment combining aristoplasty or cat surgery with other techniques depending on your concerns.
One of the most frequently ordered procedures in combination with aristocrat or cat surgery is nasal septum lengthening. Nasal septum lengthening is a procedure in which the patient's own cartilage such as auricular cartilage, nasal septum cartilage, or rib cartilage is transplanted to adjust the tip of the nose to the desired position or to extend the nasal septum itself downward. Patients who suffer from up-nose or short nose as well as protruding laugh lines and mouth should consider nasal septum lengthening in combination with aristoplasty or cataract surgery.
How to spend the time after the aristosurgery procedure
After the noble surgery procedure, it is important to avoid any activity that interferes with the wound healing process. In particular, smoking worsens blood flow and delays recovery, so we ask that you refrain from smoking for one month after the surgery. In addition, please refrain from activities that put pressure or stimulation on the affected area, such as blowing your nose strongly, sleeping on your stomach, or massaging your face. Furthermore, it is ideal to avoid activities that stimulate blood circulation, such as saunas, strenuous exercise, alcohol consumption, and dental work for about one month.
When combined with nasal septum extension, the face may not be wet until the 7th day after the surgery. Makeup can be applied on the same day, except for the area to be treated, but overall makeup will be applied only after the stitches are removed.

Frequently asked questions about nasal wing base and aristocrat surgery (prosthesis and slender rib cartilage graft)
Q&A
Can aristocrat surgery and cat surgery be performed at the same time?
Yes, a more natural and balanced result can be expected by doing them at the same time.
Will the inserted rib cartilage be absorbed?
The degree of resorption varies from person to person, but in general, tissue remaining after about 3 to 6 months is expected to be stable and settled thereafter.
Is there pain involved in the aristocrat and cat surgery procedures?
No pain will be felt during the procedure as anesthesia is administered. General anesthesia is also available, depending on your preference and the procedure, so please consult with us if you are concerned about pain. After the anesthesia wears off, you may feel a dull ache, which will subside in a few days.
Is it true that it is said that the nose widens in spite of aristocratic surgery?
The nose may appear wider after the nasal wing base prosthesis (aristoplasty) procedure. This is due to swelling caused by the procedure, or the nose bulging outward when the nasal wing base is lifted. The nose may appear wider when smiling.
What measures do you suggest to address the problem of nose spreading?
For those whose noses appear to widen, we address this problem with a procedure that tightens the nasal wings. If swelling is the cause, it is common to wait until it goes down. The swelling may continue for some time after the swelling goes down, during which time the nose may appear longer, thicker from the lower nose to the upper lip, and the nose may appear wider. It gradually subsides over time and often looks clearer in 4 to 6 months.
What are the options for a procedure to reduce the appearance of lines?
If the lines are prominent due to sagging skin rather than a sunken nasal wing base, there are procedures such as thread lift, medical HIFU (HIFU), Volnuma, and facelift. Some people also choose hyaluronic acid injections or condense rich face (CRF) to fill in the furrows caused by the lines.
What is a prosthesis?
A prosthesis is an artificial cartilage. Medical silicone is the most common, but Gore-Tex is also available, and the base of the nasal wings may be made of the patient's own cartilage or other autologous tissue.
