Double implantation: 2-point and 4-point fixation
Double Eyelid Suture MethodWithout cutting the eyelid, suture two or four points with thread.
- Without using a scalpel, the eyelid is sutured at two or four points on the inner side to create a natural double eyelid.
- No incision means no need for suture removal, resulting in minimal downtime such as swelling and bruising.
- Even if the desired double eyelid line changes, it may still be possible to make adjustments or revert it.
Double Buried Suture Method: 2-Point Fixation and 4-Point Fixation, among others
We provide eye treatments to help you achieve your ideal look.
To achieve the eye look our patients desire, we use a virtual stick during consultations to create a simulated double eyelid line on the eyelid. This allows us to preview the ideal double eyelid width and overall facial impression beforehand, adjusting the design accordingly.
For those who wish to further enhance their eye area appearance or have other eye area concerns, procedures such as inner canthus surgery, outer canthus surgery, and eyelid fat removal may also be combined.
Additionally, while the two-point and four-point double eyelid suture methods are self-pay procedures (not covered by insurance), starting February 2024, we have also begun offering insurance-covered treatments for the eye area performed by our experienced cosmetic surgeons. Please consult with us during your counseling session, whether you seek the double eyelid suture method for aesthetic purposes or are troubled by uncomfortable symptoms around the eyes.

CHARACTERISTICSDouble Buried Suture Method: 2-Point Fixation and 4-Point Fixation
The double eyelid suture method is a procedure that creates an ideal double eyelid line without using a scalpel. Instead, a specialized needle is used to tie a special thread to the tarsal plate* located on the inner side of the eyelid. The thread is finer and more flexible than a strand of hair, made from a material designed to minimize strain on the eyelid.
The placement and number of sutures may vary depending on the desired double eyelid design and the thickness of the eyelid fat, but the procedure time is approximately 15 minutes.
Depending on the condition of the eyelid, it may be possible to remove the stitches and revise or revert the double eyelid line if your desired appearance changes.
*Tarsal plate…A cartilage-like, hard tissue located at the very edge of the eyelid.
There are two approaches to the double-buried suture method.
There are two types of double eyelid suture techniques, differing in the position where the sutures are placed. At Bianca Clinic, we recommend the appropriate technique based on the condition of your eyelids and skin, as well as your desired double eyelid line.
【Tarsal Plate Method】
The tarsal plate method is a procedure suited for individuals with minimal skin laxity. It creates a double eyelid line by suturing the eyelid skin to the tarsal plate located beneath the eyelid.
The tarsal plate is harder than the levator muscle, making it easier to achieve stable fixation.
【Elevator Method】
The levator muscle method is a procedure suitable for individuals with sagging skin. It creates a double eyelid line by suturing the eyelid skin to the levator muscle located beneath the eyelid.
Since the suture does not protrude behind the eyelid, the risk of the suture damaging the eyeball is considered low.
*Levator muscle... the muscle that raises the eyelid
Characteristics of 2-Point and 4-Point Fastening
The main differences between the two-point and four-point suture methods for double eyelid surgery include the number of suture points, fixation strength, and design flexibility. However, neither method is suitable for extremely wide double eyelid designs or eyelids with significant sagging.
【Two-Point Fastening】
This method is ideal for partially adjusting double eyelid lines or creating a double eyelid line with minimal effort. Its key feature is reduced strain on the eyelid due to fewer fixation points. However, its holding power is somewhat weaker, making it harder to create wide lines or smooth double eyelid designs.
【4-Point Fastening】
This method is ideal for creating a distinct double eyelid line or achieving specific designs such as parallel double eyelids, wide double eyelids, or natural fan-shaped double eyelids. It features relatively strong fixation and facilitates the creation of a smooth line from the inner to outer corner of the eye. However, it can place stress on the eyelid, potentially leading to slightly longer downtime with symptoms like swelling or bruising.
Benefits
MERITAchieve natural double eyelids in a short treatment time
The procedure takes only about 15 minutes for both eyes and aims to create a natural double eyelid line.
No need for suture removal, and downtime tends to be short
Since no incision is made with a scalpel, there is no need for suture removal, and downtime involving swelling or bruising is relatively short.
Even when corrections or reversals are possible
In the unlikely event that your desired double eyelid line changes, it may be possible to remove the sutures and make adjustments or restore your eyelid to its original state.

RECOMMENDED POINTS
Procedures offering greater durability and design flexibility are also available.
At Bianca Clinic, in addition to the two-point and four-point suture methods for double eyelid surgery, we also offer surgical techniques that provide greater durability and design flexibility. We can recommend the most suitable procedure based on your specific concerns and preferences. Please consult with our doctors for details.
【Double Buried Suture Method (Back Suture) B TWICE】
Using a special thread finer than a hair and without a scalpel, the levator muscle is tied directly to the eyelid (levator muscle method) without piercing the eyelid surface, creating an ideal double eyelid line. This technique, known as the "hidden suture method," is characterized by leaving no needle holes on the eyelid surface.
【Double Buried Suture Method / Cross-Link Method】
A double eyelid procedure that uses specialized needles to embed sutures from the inner side of the upper eyelid into the skin along the desired double eyelid line, then specially knots the sutures together. This creates a clear, beautiful double eyelid line and is characterized as being relatively resistant to fading among buried suture methods.
【Double Buried Suture Method: Multiple Knots】
A surgical technique involving doubling the suture thread to create a small knot for fixation. By increasing the number of fixation points and meticulously calculating the skin anchorage positions, this method reduces suture dislodgement and is considered one of the most durable techniques within the double-buried suture method.
【Double Buried Suture Method / Natural Adhesion Method】
Double eyelid surgery that promotes adhesion between the levator muscle (the muscle that lifts the eyelid) and the eyelid skin to create a double eyelid line. A specialized technique involves crossing a single medical-grade thread and securing it to the levator muscle, creating multiple fixation points.
Key Points for Reducing Downtime
Generally, swelling from the double eyelid suture method peaks around the day after to the third day following the procedure. Downtime typically subsides almost completely within about 1 to 2 weeks after the procedure, and swelling tends to fully subside around 3 months after the procedure.
To minimize downtime, thoroughly cool the eye area for 48 hours after the procedure and avoid rubbing the eye area vigorously during the downtime period.
Also, please refrain from activities that affect blood flow, such as prolonged bathing, strenuous exercise, drinking alcohol, and smoking. Aim for a diet low in salt and spices, ensure adequate hydration, and try to sleep with your head slightly elevated.
For first-time customers and those wanting to update their design to match current trends
One advantage of the double eyelid suture method is that the original eyelid shape can be restored by removing the sutures. Among these, the 2-point and 4-point suture techniques are characterized by their simplicity and consideration for minimizing physical burden. Therefore, they are suitable procedures for first-time patients or those wishing to change their double eyelid design to match current trends.
On the other hand, precisely because it is simple, the double-buried suture method is a procedure where the skill of the performing surgeon can significantly affect the outcome and longevity. Furthermore, if the design is not thoroughly discussed during the consultation, the resulting double eyelid line may differ greatly from the patient's ideal.
Bianca Clinic's double eyelid suture method is performed by skilled physicians who meticulously aim to create the ideal double eyelid line for each patient.

Double Buried Suture Method: Frequently Asked Questions About 2-Point and 4-Point Fixation
Q&AHow long do double-stitch double eyelid procedures with 2-point fixation and 4-point fixation last?
The double-stitch method does not permanently maintain the double eyelid line. If the sutures securing the eyelid break or loosen, the eyelid will return to its original state. The duration varies from person to person and depends on the condition of the eyelid, lifestyle habits, and the specific surgical technique used.
When can I wear my contact lenses after the procedure?
To avoid risks such as inflammation or infection, please refrain from wearing glasses for at least two days after the procedure. However, if you experience significant swelling, bruising, or discomfort in your eyes during the recovery period, it is advisable to wear glasses until these symptoms subside. Please consult with us if this applies to you.
Can I undergo surgery while wearing eyelash extensions?
Treatment can be performed while wearing the device, but please be aware that it may come off during the procedure.
Can you accommodate the desired width and design?
We will certainly discuss your preferences, but as this depends on the condition of your eye area, please consult with us during your consultation.
Can you create double eyelids with a width of 7mm or more?
The possible double eyelid width depends on the condition of your eyelids and the overall balance of your face, so please ask the doctor during your consultation.
Can I get double eyelid surgery again even if I've had it before?
In many cases it is possible, but please consult with us during a counseling session.
If you've had surgery before or are undergoing revision surgery, do you need to have the stitches removed?
This will depend on the condition of your eye area, so the decision will be made after a doctor examines you during the consultation.
Is the thread used in the double-buried suture method harmless to the body?
Yes. Please rest assured that it is medical-grade thread. The thread used is finer than a strand of hair and is a non-absorbable suture—the same type used to suture blood vessels.
At what age can I get double eyelid surgery?
Minors may receive treatment provided they are accompanied by a parent or legal guardian and submit a consent form signed by the parent or legal guardian.
