CHARACTERISTICSMandibular prosthesis
Mandibular prosthesis surgery involves making an incision inside the mouth to insert a silicone prosthesis into the lower jaw, thereby reshaping the chin tip and facial contour. This procedure is suitable for individuals concerned about a protruding mouth, maxillary protrusion (protruding mouth), or mandibular retraction (receding jaw), as well as those seeking to achieve a more aesthetically pleasing facial contour.
A prosthesis refers to a medical-grade artificial cartilage made of silicone, possessing properties similar to human cartilage. Crafted from highly durable materials resistant to deformation and degradation, it is widely used in aesthetic medicine. While concerns like "it might look unnatural" or "it might feel like a foreign object" are often associated with prosthesis insertion—not just for the jaw—the actual result offers a natural feel and appearance, delivering a seamless finish.
The incision area is small, and the scar is inconspicuous.
The mandibular prosthesis is inserted through an incision inside the mouth, not on the visible outer part. Therefore, there is no concern about visible scarring. Those who wish to keep the prosthesis discreet can undergo the procedure with peace of mind.
Furthermore, the incision area is small, only about 2 to 3 cm, making it possible to minimize post-operative swelling and bruising as much as possible. Even when opening the mouth wide, the scar is barely noticeable. The oral cavity also has the property of healing swelling and wounds more quickly compared to other parts of the body.
【Important Considerations for Post-Lower Jaw Prosthesis Insertion】
・Shower: On the day of surgery, only the area from the neck down may be washed. Full-body showers are permitted starting the next day. Keep the fixation tape dry for 3 days. Hair washing is also permitted starting the next day. Bathing is possible starting on the 3rd day after surgery.
・Cleansing: On the day of the procedure, only wipe-off cleansing is permitted. Regular cleansing may resume the following day.
・Makeup: Can be applied starting the next day. The tape must remain in place for 3 days.
・Brushing teeth: Possible if avoiding the wound (Take care to avoid using the "i" or "u" mouth positions as much as possible)
※ Continue gargling with iodine solution for one week to keep the wound clean.
・Please refrain from opening your mouth wide or pulling your lower lip, as this puts strain on the wound.
・Avoid using straws, as pursing your lips can also be taxing.
・Please refrain from facial massage, dental treatment, and sleeping on your stomach for one month.
・Avoid activities that increase blood flow, such as drinking alcohol, long baths, or strenuous exercise, for one week, as they may cause significant swelling or bruising.
・Please refrain from smoking for one month.
We also handle removal of mandibular prostheses.
At Bianca, we also perform removal of silicone implants after insertion (we can also handle implant removal procedures for patients who had their implants inserted at other clinics). For example, if you had implants inserted but the results differ from your ideal outcome, or if you experience deformation, displacement, or movement of the implants after insertion, implant removal may be appropriate. Especially if you experience pain or swelling after surgery and suspect an infection, we will perform removal promptly, so please notify us immediately.
However, as a fundamental premise, silicone prostheses will not deform or deteriorate unless subjected to strong impact from accidents or similar events. At Bianca, we also take thorough precautions against risks such as infection when inserting prostheses into the lower jaw.
Additionally, if the size of the mandibular prosthesis does not fit, it can be removed and replaced. As with insertion, removal is performed by making an incision inside the mouth, so the scar is virtually invisible externally.
Benefits
MERITAchieve a beautiful facial contour and E-line
Custom-made silicone blends naturally to beautifully contour your jawline and profile.
Insertion through the oral cavity minimizes visible scarring
Because the prosthesis is inserted through a small incision made inside the mouth rather than from the outside, the scar is barely noticeable.
Long-term effects can be expected from a single surgery.
Silicone-based mandibular prostheses, which are resistant to deformation and deterioration, are not absorbed by the body after insertion, ensuring long-term effectiveness.

RECOMMENDED POINTSWhat are common jaw concerns among Japanese people?
Compared to Westerners, many Japanese people tend to have smaller mandibular bones or a tendency toward a receding jaw. These jaw characteristics can significantly impact facial appearance, making the face look elongated or the mouth area appear loose. While a smaller jaw can offer desirable effects like a more delicate, refined facial structure or a smaller-looking face, being too small can sometimes create the appearance of protruding teeth or a double chin. Lower jaw prosthesis insertion addresses these common jaw concerns among Japanese individuals and is a procedure offering long-term results.
Differences from Hyaluronic Acid Injections into the Jaw
These two have the following differences:
<下顎プロテーゼ>
・Since it is not absorbed into the body, its effects last long-term.
・The surgery takes approximately 30 minutes.
・Involves incisions, so there is downtime such as pain and swelling
<Hyaluronic Acid Injection>
・It is gradually absorbed into the body over a period of six months to two years, so regular re-injection is necessary.
・Treatment time is approximately 10 minutes
・Relatively little pain and downtime
Both treatments have their advantages and disadvantages, so please choose based on your lifestyle and desired results. Since the price ranges differ, feel free to consult with us if you're unsure.
Combination with other procedures around the nose and mouth is also possible.
At Bianca, we can also combine lower jaw prosthesis insertion with procedures around the nose and mouth. For example, we offer combinations with the following procedures tailored to each patient's requests:
・Jaw Botox
・Nasal implant insertion (rhinoplasty)
・Nostril Reduction Surgery
・Hyaluronic acid injections for the lips
During your lower jaw prosthesis consultation, we will propose an ideal balance that incorporates other facial features while examining your facial contour from the front and your E-line profile. If you are interested in combination treatments, please discuss this during your consultation.

Treatment Process
CONTENT OF TREATMENTPhysician's examination and counseling
During your consultation, we will discuss your concerns and desired results, followed by an examination by our physician. Please feel free to ask any questions or express any anxieties you may have about the procedure. Also, please let us know if you wish to combine this treatment with other procedures.
Design
Based on your preferences discussed during counseling and considering the overall facial balance, we create a custom-made mandibular prosthesis. First, we visualize the optimal line for your bone structure using 3D simulation (VECTRA®). Then, based on the doctor's physical examination, CT scans, and X-rays, we determine the ideal shape and thickness. Based on the design, we either select and adjust from several types of ready-made prostheses or create a semi-custom prosthesis. By meticulously fine-tuning the design using tools like 3D simulation (VECTRA®), we aim to achieve your ideal outcome.
treatment
For mandibular prosthesis procedures, local anesthesia or nitrous oxide anesthesia is administered. Relaxation anesthesia or intravenous anesthesia can also be used upon request (the same anesthesia is administered for prosthesis removal). The procedure time is approximately 30 minutes. For prosthesis removal, it takes about 20 minutes.
Follow-up
After the procedure, keep your jaw taped for 3 days (24 hours if the prosthesis was removed). You may remove the tape yourself afterward. For both prosthesis insertion and removal, sutures inside the mouth will be removed 2 weeks post-procedure.
Additionally, Bianca thoroughly monitors post-operative progress and addresses any complications. We check the condition of the incision site inside the mouth and the stability of the prosthesis, providing long-term follow-up care. If you experience any abnormalities after surgery, such as severe swelling or pain, please notify us immediately.

Frequently Asked Questions About Mandibular Prostheses
Q&AHow long does the pain last after lower jaw prosthesis surgery? Also, are there ways to relieve the pain when it occurs?
Individual results may vary, but the pain typically subsides within about a week. Antibiotics and pain medication will be prescribed, so please rest assured.
Are there any risks such as infection or changes in sensation after mandibular prosthesis insertion?
Prostheses are made from materials with a low risk of infection, but the possibility of causing an infection is not zero. Other reported complications include wound dehiscence, difficulty opening the mouth, and allergic reactions to the prosthesis.
Additionally, if small nerves are partially damaged during surgery, temporary or permanent numbness or tingling may occur in the jaw or lips. However, when performed by an experienced surgeon, the procedure can be conducted with careful consideration of these risks. Furthermore, any numbness symptoms will gradually subside over time.
Is bone resorption possible after mandibular prosthesis insertion?
Following surgery, persistent pressure on the bone may cause some degree of bone resorption (the physiological process of bone breakdown and rebuilding). With a silicone prosthesis of appropriate size and shape, adverse effects from bone resorption are highly unlikely even over many years. However, if the size or shape is unsuitable, bone resorption may cause changes such as a depression at the insertion site.
Is follow-up care required after lower jaw prosthesis insertion?
For follow-up observation, please visit our clinic at intervals of 1 to 2 weeks, 1 month, 3 months, and 6 months after surgery.
Is it possible that sleep apnea syndrome could be improved by inserting a mandibular prosthesis?
While not a direct medical treatment, in cases of mild obstructive sleep apnea involving a receding lower jaw, advancing the jaw forward may help maintain airway space and potentially alleviate symptoms. However, this does not guarantee symptom improvement.
Does a mandibular prosthesis affect orthodontic treatment or dental procedures?
If orthodontic appliances are attached to the outer surface of the lower teeth, suture failure is more likely to occur, so it is preferable to wait until orthodontic treatment is complete. Surgery is possible if you can open your mouth about two to three finger widths. We will determine your suitability for surgery during your consultation, so please discuss this with us.
Is 3D simulation (VECTRA®) available for jaw prostheses? I'd like to visualize the final result before undergoing the actual surgery.
Prior to surgery, we utilize 3D simulation (VECTRA®) to allow you to preview the anticipated results from both the side and front views before proceeding with the procedure.
Is it safe to undergo X-ray, CT, or MRI scans after having a mandibular prosthesis inserted?
You can undergo X-ray, CT, and MRI examinations without issue, but the prosthesis will appear in the resulting images.
