The midface lift is a cosmetic surgical procedure that raises the drooping midface with soft tissue and fixes it to the periosteum, smoothing it from the lower eyelid (lower eyelid) to the cheeks and giving it a youthful, balanced appearance.

There are multiple techniques for midface lifts, including approaches from the crease of the lower eyelid as well as from the temples and intrafacial incisions, depending on the facial conditions and goals of the patient. All of these techniques are expected to reduce the appearance of sagging cheeks, gorgon lines, and laugh lines by pulling up the appropriate layers of fat and supportive tissue in the cheeks, and to give a cleaner look to the midface from under the eyes to the nose.

Because it directly approaches the tissues that cause sagging, such as skin, muscle, and fat, it is also characterized by its potential for long-term lifting effects. The incision site varies depending on the surgical technique, but in all cases, the incision is placed in a position that is less noticeable.

The supraperiosteal and subperiosteal midface lift is a technique that raises the midface by approaching from the crease of the lower eyelid.

・Midface Lift (Superperiosteal) … A surgical technique where the periosteum is dissected from the lower lash line upward to elevate cheek fat, which is then sutured and fixed to the orbital periosteum. Like the open Hamra technique, an incision is made along the lower lash line.
Mid facelift (subperiosteal): The anterior surface of the zygomatic bone is dissected from beneath the periosteum and sutured together with the periosteum of the suborbital margin, the orbital septum, and orbital fat. It requires a wider dissection than the supraperiosteal dissection. It is characterized by its high strength and resistance to retroversion.

These techniques can be used in combination with the superficial hamula technique, and are characterized by their ability to simultaneously approach dark circles and depressions under the eyes.

In the superficial hamula technique, the orbital fat under the eyes is moved to the depressed area, and the entire cheek is lifted with a midface lift to create a unified look from the eyes to the midface. This approach is suitable for those who are concerned about midface drooping and dark circles at the same time.

The dual-plane midface lift is a technique that is approached through temple or intrafacial incisions to detach the cheek fat in two layers (dual plane), supraperiosteal and subcutaneous, and lift the fat body itself in a three-dimensional manner. The mid-face is lifted not only on the skin surface but also in conjunction with deeper structures, resulting in a natural three-dimensional and stable lifting effect.

This technique can be considered a development of the ponytail lift that is currently widely performed in Japan, and the midface elevation in the deep plane facelift that has become mainstream in recent years is also based on this concept. In many cases, the lower eyelid incision is not required, and it is often chosen by those who want a firm lift of the midface while taking scarring and downtime into consideration.
