Subcision is a procedure that gently separates hardened fibrous tissue beneath the skin caused by acne scars using a specialized needle, providing support from within to areas with crater-like depressions. It is suitable for noticeable acne scars and crater-like skin, and can be expected to smooth the skin's surface.
When acne inflammation becomes chronic, fibrosis progresses deep within the skin, causing tissue to thicken and harden, resulting in crater-like depressions. Subcision precisely releases these fibrous adhesions, leading to improved skin texture. At Bianca Clinic, we use subcutaneous anesthesia, so those concerned about pain can rest assured. Types of Acne Scars
There are four types of acne scars: inflammatory erythema, post-inflammatory hyperpigmentation, atrophic scars (pits), and hypertrophic scars (keloids).


Types of Atrophic Scars (Craters)
Atrophic scars, also known as acne scar craters, come in three types: icepick, rolling, and boxcar.
■Ice Pick Type
Ice-pick-type scars are small, punctate depressions measuring less than 2–3 mm in diameter. They extend from the deep dermis to the subcutaneous fat layer. Approximately 60–70% of atrophic scars are classified as ice-pick-type scars.
■Rolling Type
Rolling-type scars are craters with a diameter of 3mm or more and a gentle depression. Fibrous tissue pulls beneath the skin, resulting in varying depths. Approximately 15 to 25 percent of atrophic scars are classified as rolling-type craters.
■Boxcar Type
Boxcar-type craters are well-defined, vertical depressions with diameters ranging from 1.5 to 4 mm. Those with depths of 0.1 to 0.5 mm are classified as shallow craters, while those exceeding 0.5 mm are classified as deep craters. Boxcar-type craters are estimated to account for approximately 20 to 30 percent of atrophic scars.
Subcision (crater treatment) is a procedure suitable for rolling-type scars. Since many cases involve mixed scar types and distinguishing them can be difficult, we recommend first consulting a doctor about your acne scar concerns.

Subcision is a procedure that cuts away hardened scar tissue (fibrotic areas) deep within the skin, promoting the reconstruction of surrounding healthy tissue. Anesthesia
Subcutaneous anesthesia is administered at the site where the subcision needle will be inserted. Nitrous oxide anesthesia is also available as an option. If you are concerned about pain, please consult your physician.
