鼻整形後、
「鼻が高くなったのに鷲鼻っぽく見える」
「横から見ると鼻先の少し上が盛り上がっている」
こうした違和感として相談される代表例が、ポリビーク変形（polly beak deformity）です。
名前の通り、オウムのくちばしのように鼻が見える状態を指します。
一見すると「鼻先が下がった」「デザインが崩れた」と感じやすいのですが、In reality鼻尖そのものではなく、その直上の構造に原因があるThat's mostly the case.

■ ポリビーク変形はどこが膨らんでいるのか
ポリビーク変形で問題となるのは、鼻尖直上のsupratipと呼ばれる部位です。
本来このエリアは、鼻背から鼻尖に向かって、ストレートもしくはやや凹んだカーブdraws.
しかし何らかの理由でここにボリュームが残ると、鼻先よりも上が目立ち、結果として「鼻先が下がった」「重たい」印象になります。

■ 鼻尖縮小術で起こりやすい理由
ポリビーク変形は、とくに鼻尖縮小術後に見られることが多いです。
鼻尖縮小術は、脂肪・軟骨・支持構造を同時に扱う繊細な手術であり、わずかなバランスのズレが形態に影響します。
原因として多いのは、次の3点です。
① 脂肪や軟骨の切除量のコントロール不足
切除が不十分でも、逆に行き過ぎても、どこかに相対的なボリュームが残ります。
その結果、supratipが盛り上がって見えることがあります。
② 大鼻翼軟骨を中央に寄せすぎている
鼻尖を細くする目的で鼻翼軟骨を強く寄せると、下は細くなる一方、上の皮膚や軟部組織が被さるように前に出てきます。
正面では問題なく見えても、側面で不自然さが出やすい典型例です。
③ 設計を伴わない糸操作
鼻の皮膚の厚みや軟骨形態を考慮せず、単に糸で引き寄せるだけの施術では、立体的な破綻が起こりやすくなります。

■ 構造的要因と組織反応
ポリビーク変形の背景には、軟骨支持の不足と組織反応（瘢痕・浮腫）の両方が関与します。
支持が弱いと、上の皮膚・軟部組織が前に被さり、supratipが強調されます。
また皮膚が厚い方や剥離範囲が広い手術では、線維組織が集まりやすく、治癒過程そのものが形に影響することもあります。

■ 術後すぐの膨らみとポリビークの違い
術後早期に見られるsupratipの腫れは、多くの場合、単なる浮腫や炎症です。
ポリビーク変形として問題になるのは、腫れが引く時期を過ぎても膨らみが固定している場合is.
目安は術後3〜6か月以降。
早い段階で失敗と判断する必要はありません。

■ 修正と予防の考え方
修正の方針は原因によって異なります。
瘢痕や浮腫が主体であれば、経過観察で改善することもあります。
一方、構造的な問題が原因の場合は、鼻尖支持の再構築やsupratipのボリューム調整が必要になります。
What's important is,削るべきか、支えるべきかを見極めることis.

■ Summary
ポリビーク変形は、鼻整形後に起こり得る代表的な形態トラブルの一つですが、単純なデザインミスとは限りません。
鼻尖支持、皮膚の厚み、治癒反応など、複数の要素が重なって生じる結果です。
「鼻先の形がなんかおかしい」と感じたときこそ、鼻先だけでなく、その一段上の構造に目を向けることが重要です。
正確な評価と適切な対応によって、ポリビーク変形は十分にコントロール可能な変形といえます。
