CHARACTERISTICSPterygium Surgery: The Incision Method
Pterygioplasty is a procedure that involves making an incision in the eyelid skin and shortening the muscle that opens the eye (the levator muscle) to help support easier eye opening. At Bianca, we employ a surgical technique called the "levator aponeurosis advancement procedure." An incision is made along the double eyelid line on the upper eyelid. The levator muscle (the muscle that opens the eye) or the levator aponeurosis (the part connecting the levator muscle to the eyelid) is shortened. This helps support easier opening of the eyes.
In cases where the non-surgical method is difficult to apply—such as thick, fatty eyelids or severely drooping eyelids—the surgical method may be an option. Eyelid ptosis surgery involves making an incision in the eyelid, which offers the advantage of allowing multiple eye area procedures to be performed simultaneously while addressing the ptosis. Procedures like double eyelid creation or eyelid skin tightening can sometimes be combined. If you are interested, please consult with us.
What is MT Method for Ptosis?
The Müller Muscle Tacking (MT) method for ptosis is a procedure that uses ultra-fine medical sutures to suture and shorten the muscle tissue behind the eyelid, supporting easier eye opening. This technique is suitable for individuals with double eyelids who feel their eyes lack impact, those who wish to avoid scarring, and those seeking to minimize post-operative swelling. The MT method for ptosis does not involve cutting the skin, resulting in relatively less swelling after the procedure and minimal scarring.
One advantage of the MT method is that it allows for fine-tuning the double eyelid width during the procedure while confirming the results with the patient, making it easier to achieve the ideal look. If there is significant eyelid drooping and skin laxity, an incision method may be necessary.
[Incision Method vs. MT Method] Consult your doctor to determine which is appropriate.
If you're unsure whether the incision method or the MT method is more suitable for your ptosis, please feel free to consult with us. Our doctors will examine you and make recommendations. For your reference, suitability can be considered based on the following points:
【Candidates Suitable for the Incision Method】
・Severe ptosis
・I tried MT therapy before but didn't feel any effect ・The results didn't last long
・Significant skin laxity
【Who the MT Method is Suited For】
・I'm reluctant to make an incision
・Unable to take time off
・Try the MT method and see if there's any change (since it can be reversed by removing the stitches)
・Individuals with minimal skin sagging up to around age 50
・Desire wider double eyelids while improving eye opening

Recommended Features
RECOMMENDED POINTSPtosis that you might not notice yourself
Eyelid ptosis can be congenital or acquired, and in the early stages, individuals may not realize they have it. Initial symptoms can sometimes include seemingly unrelated issues like forehead wrinkles, stiff shoulders, insomnia, or headaches. Eyelid ptosis treatment may address these concerns, so please consult us if you have any worries.
【Classification of Ptosis】
1. Congenital ptosis
Cases where the muscles responsible for raising the eyelids are underdeveloped from birth, and instead contain a large amount of hard, inflexible fibrous tissue.
2. Eyelid drooping that develops in adulthood (acquired)
Ptosis is also common among long-term contact lens wearers and those with a habit of rubbing their eyes. While the strength of the muscles that lift the eyelid is generally not the issue, it is thought to be caused by weakness, detachment, or holes in the tendon attached to the eyelid.
3. Symptoms resembling ptosis (pseudoptosis)
This condition involves drooping eyelids, a narrowed appearance, or sunken eyes that make the eyelids appear lowered. Since the muscles and tendons that lift the eyelids are normal, it is referred to as pseudo ptosis.
Is ptosis covered by insurance? Key points for distinguishing it
Eyelid ptosis may be covered by insurance in certain cases. The difference between insurance-covered treatment and private treatment (not covered by insurance) is as follows. Bianca Clinic offers both insurance-covered and private treatment options. Whether you qualify for insurance coverage will be determined by a doctor's diagnosis, so please consult with us first.
【Cases Subject to Out-of-Pocket Treatment】
・The purpose is to pursue aesthetic qualities such as visual beauty and design appeal.
・Aiming to pursue both functionality and aesthetics
【Cases Where Insurance Coverage May Apply】
・Accompanied by visual field defects or functional impairments
・The primary objective is the pursuit of functional improvements
・Congenital ptosis
Combined with other treatments, achieve your ideal eye area look
During ptosis surgery, it can be combined with other procedures to comprehensively address concerns around the eyes.
【Examples of Treatments That Can Be Performed Simultaneously】
・Under-eye dark circles and sagging removal
Procedures to remove dark circles and sagging under the eyes include the Hamra method, reverse Hamra method, and lower eyelid lift. At Bianca, we propose customized treatment plans combining procedures based on the cause of your concerns.
・Inner Canthus Surgery
A procedure that involves making an incision in the area called the epicanthal fold at the inner corner of the eye to reveal the tear mound (the pink area at the inner corner). By widening the eye, it can enhance the eyes' expressiveness.
・Hyaluronic acid injection
If you're concerned about hollows under your eyes or eyelids, hyaluronic acid injections can be used to address this. By carefully considering your bone structure and the position of your double eyelid line while injecting an appropriate amount, we can aim for a natural, youthful-looking eye area.
Whether blepharoplasty can be combined with ptosis surgery and which procedure is appropriate will be determined by the doctor during an examination. Please consult with us during your consultation for details.

Eyelid Ptosis: Common Questions About Incision Method and MT Method
Q&AWhich is better: blepharoplasty or the MT method?
Treatment methods for ptosis vary depending on the severity of symptoms. For severe ptosis with significant skin laxity, the incision method is often the recommended approach. For those hesitant about incisions or individuals under 50 with minimal skin laxity, the MT method may be a viable option. We will propose a treatment plan after examining your eye condition during a consultation. Please consult with our physician for details.
Can ptosis return after surgery?
【MT Method】
This method involves suturing the muscle to shorten it without making an incision. As long as the sutures remain intact, the muscle will not return to its original state. However, if the sutures break or become detached from the tissue, there is a possibility the muscle could revert to its original position.
【Incision Method】
While it generally does not revert to its original state, there is still a possibility of reversal if you rub or pull on your eyelids too hard during the first six months after the procedure.
How long does it take for the results of blepharoplasty to stabilize?
It takes approximately 6 months for the condition to fully stabilize. Significant swelling typically subsides within 1 to 2 weeks and largely resolves after about 1 month. While individual results vary, temporary widening of the double eyelid crease or asymmetry may occur, but the final result should approach completion over approximately 3 to 6 months.
Is the thread used in the MT method for ptosis safe for the body?
The sutures used in the MT method are non-absorbable sutures that are not absorbed by the body. They are thinner than a strand of hair and are the same medical-grade sutures used for suturing blood vessels, ensuring safety.
Can I get a treatment while wearing eyelash extensions?
We can perform the treatment even if you have eyelash extensions, but please be aware that some extensions may come off during the procedure.
How many days after blepharoplasty surgery are the stitches removed?
Sutures will be removed one week after the procedure.
When can I start wearing makeup after ptosis surgery?
With the MT method, makeup can be applied starting the next day. With the incision method, makeup can be applied starting the day after suture removal.
When can I start wearing contact lenses after ptosis surgery?
For blepharoplasty, you may resume use approximately one month after the procedure. For the MT method, you may resume use approximately two weeks after the procedure.
When can I wash my face after ptosis surgery?
You may wash your face starting the day after your ptosis surgery. To keep the wound clean, please wash your face gently.
What is the difference between the MT method for ptosis and the double eyelid suture method?
Both the MT method for ptosis and the double eyelid suture method involve suturing the inner eyelid with thread, but they differ in the suture location and purpose.
Ptosis Correction MT Method: This procedure aims to improve the opening of the eye by suturing the levator muscle and levator aponeurosis—muscles essential for lifting the eyelid—closer to the tarsal plate (the hard supporting tissue on the inner side of the eyelid) and then securing them in place.
Double-stitch method: The goal is to create a double eyelid line by suturing the inner side of the eyelid.
Can drooping eyelids be treated with ptosis surgery?
Eyelid ptosis surgery can also be effective for upper eyelid sagging. However, depending on the patient's eyelid condition, it may be difficult to completely eliminate sagging with ptosis surgery alone. Please consult with your doctor during your examination for details. At Bianca, we offer procedures such as the "brow lift" and "upper eyelid fat removal" that can be effective for upper eyelid sagging.
Is ptosis surgery painful?
You may feel some pain from the injection when the anesthetic is administered, but you should feel little to no pain during the procedure itself as the anesthetic will be taking effect.
Even after undergoing ptosis surgery, does ptosis recur with aging?
After ptosis surgery, age-related changes such as narrowing of the double eyelid crease due to sagging may still occur. However, patients who have undergone ptosis surgery may experience a slower progression of eyelid drooping compared to those who have not. Should ptosis become noticeable again with advancing age, repeat surgery is possible.
