[Important Notes for Use]

・For FORMA V and VTONE, you must purchase a dedicated chip when you first sign up (25,000 yen each, tax included).

・Additional fee for topical anesthesia for the Vaginal Care® injection: 3,630 yen (tax included)

・Unused treatment sessions cannot be carried over to the following month.

・Regular medical checkups are required.

・Depending on the results of a medical examination, we may not be able to provide treatment.

・There is a cap on the number of members, so we may suspend new enrollments.
