Specialist in rhinoplasty. He is a board certified plastic surgeon by the Japanese Society of Plastic and Reconstructive Surgery. Based on the principle of “maximum effect with minimum treatment,” I propose the best surgical techniques and plans for my patients from various angles. He specializes in designs that beautifully accentuate the "individuality" of the patient while taking into account the golden ratio of the face. In particular, we create full-custom-made prostheses by combining CT and Vectra data to create full-scale blueprints, which are then precisely finished based on clay prototypes handmade from scratch on a skull model. The shape of the prosthesis can be reproduced with high precision as per the presented simulation, so that even the most detailed wishes can be met and stable results can be achieved.

Hello!

I am Yu Yagasugi, "Japan's leading expert in rhinoplasty."

As a board-certified plastic surgeon recognized by the Japanese Society of Plastic and Reconstructive Surgery, I have performed rhinoplasty for many patients concerned about their noses. At Bianca Clinic, I not only provide patient care but also serve as a technical instructor.

Now, let’s talk about the “bulbous nose”—a common concern at our clinic. Are you worried, thinking, “Sure, some people with bulbous noses are cute, but what about me?”? The truth is, while a bulbous nose can be a charming feature for some, for others, having it refined can instantly give them a more sophisticated look. In this article, we’ll explain the characteristics of people who look cute even with a bulbous nose, as well as cosmetic surgery options for achieving your ideal nose.

To begin with, a "bulbous nose" refers to a nose with a round, thick tip, a feature said to be common among Asian people. People with bulbous noses who still give the impression of being "cute" are,

  • It harmonizes well with the rest of the face.
  • It exudes a charming atmosphere

It can be said that it has these characteristics.

However, if someone with a bulbous nose feels they “want to look more polished” or “want a sleeker profile,” the reason may lie in the balance of the tip’s height and angle.

When treating patients who are concerned about having a bulbous nose, my priority is not simply to alter the height or angle of the tip, but to create a design that beautifully highlights their unique features. I propose a natural-looking result based on the golden ratio of the face.

What Type of Nose Do You Have? Check Out the Different Types and Characteristics of Round Noses

Cosmetic Surgery to Refine a Bulbous Nose and Achieve a "Cuter" Look

Broad-nosed noses can be broadly categorized into

  • A type in which the cartilage at the tip of the nose (alar cartilage) spreads out to the sides
  • People with thick fat on the tip of their nose
  • The type with thick skin on the tip of the nose

There are various causes, and it is important to take an approach tailored to each one.

Therefore, in addition to the consultation, I use the VECTRA® 3D simulation system to capture images of the patient’s face and thoroughly analyze the causes of a bulbous nose. My approach is to create a precise treatment plan while using the 3D simulation to visualize the expected results after the procedure.

By reviewing a 3D visualization of the expected results with the patient prior to surgery and performing the procedure according to the surgical plan, we prevent any discrepancy between the patient’s ideal outcome and the actual result.

While there are various types of cosmetic surgery procedures to address a bulbous nose, in this article we’ll explain “nasal tip reduction/nasal tip reshaping (bulbous nose correction),” which addresses the root cause of the condition.

What Is Nasal Tip Reduction/Rhinoplasty (Correction of a "Round Nose")?

鼻尖形成を説明する画像
Nose tip reduction is a procedure that reshapes the tip of the nose into a sleeker form by removing fat from the nose tip and pulling together and securing the alar cartilage (the cartilage that forms the curve from the tip of the nose to the nostrils). The goal is to achieve a nose that appears slender and sharp when viewed from the front.

In addition, tip rhinoplasty is a procedure that involves suturing the alar cartilages together to reduce roundness, resulting in a nose with a sharp, defined appearance. By combining this procedure with grafts of auricular cartilage or septal cartilage, it is also possible to further refine the height and shape of the nose. At Bianca Clinic, we also perform the columellar strut technique, in which auricular cartilage or other materials are grafted between the alar cartilages to serve as a support, thereby adjusting the nose to the ideal height and angle.

Both nasal tip reduction and nasal tip reshaping are cosmetic surgical procedures performed to correct a bulbous nose. After determining the type of bulbous nose based on the results of a physician’s examination and imaging tests, we will propose the most appropriate treatment approach, which may involve combining this procedure with other treatments as needed.

Nose tip reduction/nose tip reshaping (correction of a bulbous nose) not only has the benefit of transforming a bulbous nose into a more refined appearance, but in some cases,

  • There is a possibility of a setback
  • The overall balance of the nose changes
  • Sometimes it can be hard to really feel the changes.

It is also true that there are drawbacks such as these.

To avoid these potential drawbacks, it is important not only to rely on the doctor’s experience and technical skills but also to thoroughly listen to the patient’s concerns and wishes before surgery. At our clinic, we place great importance on creating a welcoming atmosphere where you feel comfortable speaking openly, so please feel free to discuss anything during your consultation or examination.

[Case Study] Correcting a Bulbous Nose to Bring Out a “Natural, Cute” Look

I will explain a case of a bulbous nose that we treated at our clinic, using images as a reference.

[Procedure Details] Nasal Septum Extension, Nasal Tip Reshaping
[Image Caption] Top: Before treatment; Bottom: Immediately after treatment
[Costs] Nasal Septum Lengthening: 660,000 yen; Nasal Tip Reshaping: 348,000 yen
[Side Effects and Risks] Swelling, bruising, infection, asymmetry of the nose, relapse, nasal congestion, etc.

In these before-and-after images, the top shows the condition before the procedure, and the bottom shows the condition immediately after. Because this is a closed method that accesses the area from inside the nostrils, scars are barely visible from the outside, and swelling and bruising are virtually imperceptible to the naked eye.

Although the patient had a somewhat prominent bulbous nose, we carefully designed the procedure to create natural changes in the height and angle of the nasal tip, followed by septal extension and rhinoplasty. As a result, the nostrils are less visible when viewed from the front, and the nose appears sharper when viewed from the side. Although the change wasn’t dramatic, correcting the bulbous nose—which had been a source of concern—brought out even more of the patient’s natural charm.

Frequently Asked Questions (FAQ)

Can You Fix a Bulbous Nose on Your Own?

While massage may temporarily reduce swelling, it is medically difficult to alter the shape of the cartilage or the thickness of the skin, which are the causes of a bulbous nose. If you are concerned about a bulbous nose, please schedule a consultation to discuss your options.

Why are some people cute even with a bulbous nose?

Depending on the overall balance of the face—such as the amount of space around the nose and the placement of facial features—as well as a charming appearance, some people don’t mind having a bulbous nose.

How long is the downtime for nasal tip reduction/rhinoplasty (correction of a bulbous nose)?

Although results vary from person to person, severe swelling generally subsides in about a week, and bruising typically subsides in about 10 days.

Are the scars from surgery on a bulbous nose visible from the outside?

For procedures on the tip of the nose, there are two methods: the closed technique, which involves making an incision inside the nostrils, and the open technique, which involves making an incision along the columella—the bridge of the nose between the nostrils. The closed technique is characterized by less noticeable scarring.

Even with the open rhinoplasty technique, the scar on the columella is often barely noticeable. Since the incision site varies depending on the procedure and the shape of your nose, please schedule a consultation to discuss your options.

How many days do I need to take off from work?

After the procedure, a cast will need to be worn over the nose for about a week. Many people plan to take time off work around that time.

Is it okay to bring a photo of my ideal “cute nose” to my appointment?

Of course, you’re more than welcome to bring them. We’ll use the photos you bring as a reference, along with images captured during your consultation and via our 3D simulation system, to thoroughly discuss the nose shape that will best bring out your natural beauty.

Is there any pain during or after nose surgery?

In addition to local anesthesia, we use sedation or intravenous anesthesia upon request to minimize pain. If you are concerned about pain, please feel free to let us know.

If I have nose surgery, will the results last a lifetime?

In the case of surgery involving an incision, it is said that the results will last for a long time once the shape has stabilized. Our clinic provides thorough aftercare, so please feel free to consult with us regarding any concerns or questions you may have following your procedure.
