CHARACTERISTICS

Inner Canthus Surgery

Mongolian fold surgery is a cosmetic procedure that involves incising and adjusting the naturally occurring skin fold known as the epicanthal fold. This widens the horizontal width of the eyes, creating a larger, more striking eye appearance. The epicanthal fold refers to the skin extending from the upper eyelid toward the inner corner of the eye. By incising the inner corner, the hidden lacrimal papilla (the red area at the inner corner) becomes exposed, giving the eye shape a more defined impression. Beyond enhancing the inner corner appearance, another benefit is that it makes the double eyelid line more beautifully defined and prominent. This procedure is particularly suited for those desiring a parallel-type double eyelid or wishing to narrow the distance between their eyes, and it can also help improve the overall facial balance. At Bianca Clinic, we tailor the procedure to each patient's eye shape and ideal outcome, striving for a natural finish by minimizing the incision area as much as possible. We perform delicate procedures with careful consideration for downtime and scarring.

【Z-Plasty and W-Plasty】Customized Inner Canthus Surgery for Each Individual

There are several surgical techniques for epicanthoplasty, and the surgeon will determine the appropriate technique after examination. Here, we introduce two representative techniques.

【Z Method】
A surgical procedure involving making a Z-shaped incision at the inner corner of the eye, repositioning the skin flap, and suturing.
・Benefits:
Easy to adjust the design
Relatively less noticeable scarring and shorter downtime
If you're not satisfied with the result, it's easy to make adjustments again.
・Disadvantages:
Dramatic changes may be difficult to perceive.

【W Method】
A surgical procedure involving making a Z-shaped incision at the inner corner of the eye and excising skin.
・Benefits:
It is easier to open the inner corner of the eye wider than with the Z-plasty method.
You can easily feel a distinct change.
Suitable for use in combination with double eyelid surgery
・Disadvantages:
The time required for surgery, wound recovery, and downtime tends to be longer than with the Z-plasty method.
Once incised, it is difficult to restore to its original state.

As time passes, the scars become less noticeable.

Since inner corner blepharoplasty involves making incisions around the eyes, many people hesitate to undergo the procedure due to concerns about noticeable scarring. While individual results vary, scars typically become less visible over time. Please review Bianca's case photos to see the post-operative progress and scarring after inner corner blepharoplasty.

【Immediately after surgery】
The period until suture removal is when stitches remain around the eyes, making the wound more noticeable.
【5 to 7 days after surgery】
Once the stitches are removed, the sutures are gone and you can apply makeup, making it easier to conceal the scar.
【Approximately one month after surgery】
A slight redness may appear.
【Approximately 2 to 3 months after surgery】
The scar gradually becomes a thin white line and becomes less noticeable.
Individual results may vary, but scars become barely noticeable with makeup.

RECOMMENDED POINTS

The key to the inner canthus incision line is “not too little, not too much.”

The key to successful inner corner eyelid surgery is achieving a natural and beautiful design. If the incision line is too small, the change may be barely noticeable; if it's too large, the tear mound becomes overly visible, creating an unnatural appearance. In fact, some patients who underwent inner corner surgery elsewhere but were dissatisfied with the results come to Bianca for revision surgery.

Generally, the ideal "golden ratio of beauty" is considered to be a width ratio of 1:1:1 for the right eye, the distance between the eyes, and the width of the left eye. However, the design for epicanthoplasty cannot simply be applied based on this ratio alone. At Bianca, we determine the design by carefully assessing the overall facial balance, including the condition of the epicanthal folds and bone structure. To achieve the desired outcome, please feel free to discuss any concerns or questions you may have.

Minimizing postoperative burden through comprehensive downtime management

While epicanthoplasty is a procedure with relatively minimal downtime, some patients may feel anxious about how to care for themselves post-surgery. Paying attention to the following points during your recovery period can help reduce the burden after surgery.

【Things to Avoid During Downtime After Inner Corner Blepharoplasty】
・Long baths, alcohol consumption, and strenuous exercise: These activities increase blood flow and may worsen swelling or bruising.
・Avoid getting water in your eyes: Especially until your stitches are removed, take care to avoid any irritation around your eyes.
・Lying down for extended periods: Blood may pool around the eyes, making bruising more noticeable.

Combined with double eyelid surgery, you can achieve your ideal eye area.

Incising the epicanthal fold during epicanthoplasty not only creates a more wide-eyed impression but can also result in double eyelids for some individuals. For those whose natural double eyelid crease lies above the epicanthal fold, epicanthoplasty alone may create double eyelids. However, for individuals whose double eyelid crease lies within the epicanthal fold, epicanthoplasty alone is unlikely to create double eyelids. In such cases, combining epicanthoplasty with double eyelid surgery may make it possible to form double eyelids. Bianca offers two main types of double eyelid surgery. Please consult with us during your consultation to determine if you are a suitable candidate.

【Buried Suture Method】A technique that uses medical-grade thread without cutting the eyelid
・Both treatment time and downtime tend to be short
・Double eyelids may revert to their original state over time

【Incision Method】A method of creating double eyelids by making an incision in the eyelid
・Long-lasting effects can be expected from a single treatment
・Requires more procedure time and downtime than the buried suture method

Frequently Asked Questions About Inner Corner Blepharoplasty

Q&A

How much can I enlarge my eyes with an epicanthoplasty?

Design and adjustments can be made down to the millimeter. Depending on your preferences and the condition of your eyes, procedures like double eyelid surgery or lateral canthoplasty can be combined to achieve results closer to your ideal. Please discuss this during your consultation.

Are the scars from epicanthoplasty noticeable?

The incision made on the skin surface will gradually fade over several months, changing from reddish-brown (hyperpigmentation) to a pale line, becoming less noticeable. The scar can be covered with foundation or eye makeup to the point where it is barely visible.

Are there people who aren't suited for epicanthoplasty?

Epicanthoplasty is a procedure that reduces the distance between the eyes by cutting the epicanthal fold. Therefore, it may not be suitable for individuals who already have minimal epicanthal folds or naturally close-set eyes. If such individuals undergo epicanthoplasty, the lacrimal mounds may become overly visible, potentially creating an unnatural appearance. If you are unsure whether epicanthoplasty is appropriate for you, please consult with us during your counseling session.

After undergoing an epicanthoplasty, is it possible for the results to revert?

While the likelihood of a medial canthoplasty reversing is low, it can occur if the surgical technique is unsuitable or if the skin reattaches over time. At Bianca Clinic, we select a surgical technique tailored to each patient's eye area and perform the procedure with care to prevent a return to the pre-operative state.

Can I get an epicanthoplasty done on just one eye?

Single-eye epicanthoplasty is also possible. For individuals with asymmetry in eye shape or epicanthal folds, the procedure may be performed on only one eye to match the other.

When can I get eyelash extensions or wear contact lenses after an epicanthoplasty?

It is advisable to refrain from eyelash extensions for about one month, until the incision from the epicanthoplasty has healed. Contact lenses may be worn starting three days after the procedure.

How much pain is there during and after surgery?

During the inner canthus surgery, topical anesthetic eye drops and local anesthesia are used, so you will feel little to no pain during the procedure. If you are concerned about pain, please discuss this during your consultation.

I had a failed epicanthoplasty before. Can I get revision surgery?

At Bianca, we perform revision surgery for procedures performed at other clinics. For epicanthoplasty, some patients seek revision when the incision was too deep, resulting in an unnatural appearance, or when the desired change wasn't achieved despite the incision. Revision surgery requires a high level of technical skill, so our experienced doctors at Bianca will handle it. Bringing documentation detailing the surgery performed at the other clinic will help the process go smoothly.

Inner corner blepharoplasty or outer corner blepharoplasty—which is better?

Inner and outer canthoplasty each have distinct characteristics, and the most suitable option varies for each individual patient. At Bianca Clinic, we can perform both inner and outer canthoplasty procedures simultaneously. Please discuss this during your consultation.
