18The following regulations apply to students under the age of 18 (currently enrolled in high school).
*A parent or guardian may be required to submit a parental consent form and be accompanied by a parent or guardian for certain procedures.

■Counseling only

You may visit the clinic in person only.
There are no required documents to be submitted, but please note that we cannot accept contracts on the same day if you come to the clinic alone.
*When considering surgical procedures, we ask that a parent or guardian be present whenever possible in order to share accurate information.

■Laser and injectable treatments

You may come to the clinic in person only, but you must submit a parental consent form.

■ Surgery

You may come to the clinic alone on the day of your visit, but we ask that you be accompanied by a parent or guardian at all times during checkout.
*Parents may wait in the waiting room during the surgery.

Note regarding payment methods.

• PayPayOnly balance payments are accepted. *Please be aware of the points awarded and the maximum charge amount.

• Credit cardIn principle, the card must be in the name of the patient. If you are unavoidably coming to the clinic alone and using a credit card in the name of a parent or guardian, we will call the cardholder to confirm his/her identity and payment agreement at the time of checkout.
