[Contract Provisions]
◎Applies to all plans:
・An initial consultation with Dr. Miyamoto is required (regardless of whether it is a new or follow-up visit)
・When signing up for the first time, you must purchase FOMAV and VTONE chips separately. *Chip price: 25,000 yen (tax included) per chip
・Unused amounts cannot be carried over to the following month.

An appointment for an examination by Dr. Miyamoto is required every three months

After 3 FOMAV treatments (approximate), schedule an appointment with Dr. Miyamoto for an examination

・After 3 FOMAV treatments (as a general guideline), schedule an appointment with Dr. Miyamoto for an examination
・An additional fee of 3,630 yen for topical anesthesia will be charged for vaginal rejuvenation injections.
・Since a physical examination is performed during vaginal development treatments, the timing of all follow-up examinations is determined by the physician.
Monitor pricing and discount rates vary depending on the treatment's applicability, degree of change, and the level of exposure in photos/videos. Details regarding monitor status will be determined by the physician during your consultation.
