「まぶたが重い」「年齢より疲れて見える」
こういった上まぶたの悩みは多くの方が抱えるものですが、選ぶべき手術は症状が似ていても大きく異なります。
重要なのは、希望の変化量や好みではなく、「どこに余りがあるか」という原因です。
本記事では原因に応じた正しい手術選択について解説します。

■ 眉下切開が適しているのは「まぶた単独の余り」
眉下切開は、上まぶた自体の皮膚が余っている状態に効果を発揮します。
✔︎ 本来の二重ラインが皮膚の余りで隠れてきた
✔︎ まぶたに厚み(ROOFという脂肪の厚み)があり、重く見える
✔︎ 額には大きな皮膚の余りがない
この場合、まぶたそのものを処理するのが最短ルートです。
眉毛のすぐ下から余剰皮膚を取り除くため、本来の二重ラインを再び露出させることができます。
「新しい二重を作る」のではなく、隠れてしまった二重を取り戻す治療という位置付けです。
厚みが強い場合は脂肪除去を併用することで、さらにスッキリした印象に仕上がりやすくなります。

■ 前額リフトが適しているのは「額の余りが波及している状態」
一方、前額リフトは、額と上まぶたがセットで緩んできているタイプに向いています。
✔︎ 額のシワが深く刻まれてきた
✔︎ 額〜まぶたにかけて広くたるみがある
✔︎ 眉を上げて視界を確保している
✔︎ 常に疲れて見える
額の余りが下に押し寄せると、まぶたが重そうに見えてきます。
しかし、まぶただけ切除しても根本原因（額）を残すため、再発が早くなるリスクがあります。
前額リフトは額側の余りを上方向へ戻す手術のため、額＋上まぶたが同時に改善し、表情全体が明るくなります。
長期的な安定性を求める方に特に適した方法です。

■ 最重要ポイントは「皮膚の余りの発生源」
治療成否を分ける分岐点は非常にシンプルです。
・まぶただけが余っている → 眉下切開
・額の余りがまぶたに波及している → 前額リフト
逆の手術を行うと、効果が不十分 / 余計な変化 / 早期再発、といったミスマッチが生じます。
だからこそ、術式選択は原因に応じて判断すべきなのです。

前額リフトも眉下切開も優れた手術ですが、効果範囲も目的も異なります。
