鼻整形を検討する中で、「コルメラストラット法」と「鼻中隔延長のどちらが良いのか」と迷う方は少なくありません。
どちらも鼻先を支える手術ですが、構造の作り方と安定性に明確な違いThere is.
この違いは、変化量と長期的な安定性に大きく関わってきます。

■ コルメラストラット法とは？
コルメラストラット法は、鼻先の下に軟骨で「柱（ストラット）」を立て、鼻先を下から支える手術です。
鼻先の角度調整や軽度の位置修正に向いており、比較的シンプルな方法といえます。
多くのクリニックで行われているコルメラストラット法は、鼻中隔に固定せず、鼻中隔前方の大鼻翼軟骨内側脚の間に柱を添える floating strutis.
floatingとは「宙に浮いている」という意味で、鼻中隔という支持基盤を持たない状態を指します。
この方法でも、手術直後はある程度、鼻先の形を整えることが可能です。

一方で、移植した軟骨を鼻中隔軟骨などにしっかりと縫合固定する方法は、fixed strutと呼ばれます。
fixed strutは、より大きな変化（鼻先の投影や回転）を出したい場合や、不安定な鼻先を強力に補強したい場合に選択されます。
その反面、構造が強固になる分、鼻先が硬くなる傾向があります。
コルメラストラットを、構造的に鼻中隔延長に近づけた方法といえます。
術後の安定性を重視して、コルメラストラットを fixed strut として用いるドクターもいます。
私自身も、コルメラストラットで行う場合には、fixed strutで行うことが多いです。

■ 鼻中隔延長との決定的な違い
鼻中隔延長は、移植した軟骨を既存の鼻中隔に直接固定し、支持基盤のある骨組みそのものを作り直す手術です。
そのため、鼻先の位置・向き・安定性を、より構造的にコントロールすることが可能です。

一方、floating strutによるコルメラストラット法は、鼻中隔に固定されていないため、柱が遊離した状態となります。
このため、手術直後は形が整っていても、時間の経過とともに、わずかな沈みや角度変化が生じることがあります。
構造の違いを整理すると、
・コルメラストラット法（floating）：柱を「添える」補助的な支え
・コルメラストラット法（fixed）：固定によって支持力を高めた方法
・鼻中隔延長：骨組みそのものを「再構築」する方法
このような違いになります。

■ どちらを選択するべきなのか｜構造と自然さのバランス
鼻先は、笑ったり話したりと、日常の表情の中で常に動く部位です。
そのため、構造をどこまで強固に作るかによって、仕上がりの質感や動き方は大きく変わります。
nasal septum extensionは、しっかりとした骨組みを作ることで、鼻先の位置や向きを大きく、かつ自由度高くデザインすることができます。
一方で、構造が強固になる分、動きは出にくくなり、鼻先に硬さを感じやすいという側面もあります。
彫刻のように形を作り込む手術、と表現すると分かりやすいかもしれません。
それに対してCormer-Lustrat methodは、変化量やデザインの自由度では鼻中隔延長に及びませんが、鼻先の可動性が保たれやすく、表情に馴染んだ自然さを出しやすいHere's how.
どちらが優れているという話ではなく、「どこまで変えたいのか」「自然さと変化量のどちらを重視するのか」によって、適した方法は変わってきます。

■ Summary
コルメラストラット法と鼻中隔延長は、鼻先を支えるという点は同じでも、構造の作り方が異なります。
変化量を大きく、安定して出したい場合は鼻中隔延長が適しており、控えめな変化かつ自然な動きや質感を重視する場合はコルメラストラット法が向いています。
fixed strutを用いることで、コルメラストラットでも安定性を高めることは可能です。
どこまで変えたいのか、何を優先したいのかによって、選ぶべき方法は変わります。
