Abdominoplasty is a cosmetic surgical procedure that uses liposuction to adjust the thickness of subcutaneous fat in the abdomen and accentuate the vertical lines of the abdominal muscles. Rather than sculpting the muscles themselves, the procedure aims to create a three-dimensional abdominal contour by removing fat.

Have you ever thought, “I want to develop natural vertical lines on my stomach,” or “Even when I diet, my abs don’t show up clearly”?

The appearance of the abdomen is influenced not only by the amount of fat but also by the shape of the muscles and the condition of the skin. Abcracks is a procedure that uses liposuction to sculpt the abdominal contours, taking these anatomical factors into account.

In this article, BIANCA CLINIC explains how abcracks are formed, the results you can expect, and their pros and cons.

What kind of procedure is AbCracks?

Abdominoplasty is a cosmetic and plastic surgery procedure that involves selectively removing fat from the abdomen to accentuate the vertical lines of the abdominal muscles.

We apply a technique called “Abdominal Etching,” which adjusts the thickness of fat along the contours of the abdominal muscles, to achieve a sculpted abdominal silhouette.

"Ab cracks" refer to the vertical lines that appear on the abdomen.

"Abcracks" is a portmanteau of "ABS," which refers to the abdominal muscles, and "CRACK," which means a groove. Generally, it refers to the appearance of vertical lines on the abdomen.

In the abdomen, a pair of muscles called the “rectus abdominis” run vertically from just below the chest to the pubic bone. Located in the center of these muscles is a band of connective tissue called the “linea alba,” which connects the left and right rectus abdominis muscles.

The appearance of the vertical lines of the abdominal muscles depends not only on the relative positions of these muscles and connective tissues but also on the thickness of subcutaneous fat and the condition of the skin.

That's why, even when people follow the same workout routine, some have clearly defined abs while others don't.

How does the "Ab Cracks" exercise create vertical lines in the abs?

The AbCracks procedure does not alter the muscles themselves; rather, it involves suctioning the subcutaneous fat above the abdominal muscles and adjusting its thickness to accentuate the vertical lines.

At Bianca Clinic, we assess the position of the abdominal muscles and skeleton and design the procedure based on the following five reference points.

  • Left and right waist: The area that forms the waist, extending from below the ribs to above the pelvis
  • Half-Moon Line: The line running along the outer edge of the rectus abdominis muscle. Check its location while engaging your abdominal muscles.
  • Midline: The line down the center of the abdomen, extending from the navel toward the solar plexus
  • Inguinal triangle: The area located just above the groin that forms the line leading to the waist
  • Upper Chest Triangle: The area near the ribs where the serratus anterior and external oblique muscles meet

After confirming these locations, we will perform liposuction to remove subcutaneous fat.

The key here is not to leave fat in place to create a bulge, but to adjust the fat in the superficial layers as well, making the area that forms the vertical line even thinner than the surrounding tissue.

For example, when creating a vertical line down the center of the abdomen, we adjust the fat along the midline to make it thinner, creating a shadow that resembles a groove. By utilizing these differences in fat thickness, we aim to create a three-dimensional effect that follows the structure of the abdominal muscles.

At Bianca Clinic, we use VASER® for liposuction.

At Bianca Clinic, we use the “VASER®,” an ultrasound-assisted liposuction device, for abdominoplasty.

VASER® is a device that uses ultrasonic vibrations to treat fatty tissue, making it easier to suction out.

In abdominoplasty, it is not only necessary to reduce fat throughout the entire abdomen, but also to selectively adjust the thickness of the fat along the contours of the abdominal muscles. Therefore, it is important to carefully determine the layers and areas from which fat will be removed.

At Bianca Clinic, VASER® Lipo-certified physicians determine the extent and depth of fat removal based on the anatomical structure of the abdomen. We prioritize a design that considers the overall balance—not just the amount of fat removed—to ensure the abdominal muscles appear natural.

However, using VASER® does not guarantee that you will achieve your desired contour. The final result also depends on the natural shape of your muscles, the condition of your skin, and the distribution of fat.

What results can I expect from abcracks training?

With abdominoplasty, in addition to accentuating the vertical lines of the abdomen, you can expect to improve the overall contour of the abdomen by adjusting the distribution of fat.

However, since the shape of the abdominal muscles and the condition of the skin vary from person to person, not everyone will experience the same changes.

Highlight Your Vertical Ab Lines and Achieve a Toned, Defined Stomach

The main purpose of creating "ab cracks" is to create shading that follows the structure of the abdominal muscles and accentuate the vertical lines.

Whether or not you can see vertical lines on your stomach isn't determined solely by the development of your abdominal muscles. The thickness of the fat layer over the muscles is also one of the factors that affects your appearance.

By adjusting the fat distribution to match the contours of your abdominal muscles, you may be able to create a three-dimensional appearance in your midsection without changing the muscles themselves.

However, you cannot freely determine the depth or appearance of the vertical lines; the design must be tailored to the original structure of the abdomen.

It can also help sculpt the overall contour of your abdomen

To make your vertical abdominal lines stand out, it's important to consider not only the central area but also how fat is distributed across your entire abdomen.

For example, if you have a lot of subcutaneous fat in your lower abdomen or around your waist, emphasizing only the vertical lines can make the connection with the surrounding contours look unnatural.

For this reason, when performing an abdominoplasty, we determine the area to be treated for liposuction while considering the balance from the center of the abdomen to the waist. Another key feature of this procedure is that by refining the contour of the entire abdomen, we can achieve a body silhouette where the vertical lines and the waist blend seamlessly.

If you have sagging skin, it may affect the final result.

In abdominoplasty, the condition of the skin after fat removal also affects the final result. If the skin has sufficient elasticity, the abdominal contour can be expected to adapt naturally as fat is reduced.

On the other hand, if excess skin has developed due to significant weight fluctuations or pregnancy, sagging may persist even after fat is removed. Furthermore, if fat is removed before the skin has sufficiently contracted, it is possible that unevenness may become noticeable.

When considering the contours of the abdomen, it is important to assess not only the amount of fat but also the thickness and elasticity of the skin, as well as the degree of sagging.

Has the effectiveness of AbCracks been medically confirmed?

Regarding “Abdominal Etching,” which is related to the formation of abdominal cracks, medical studies conducted overseas have examined abdominal contouring and postoperative satisfaction.

In addition, based on our clinical experience at Bianca Clinic, we presented on abdominoplasty at the Japanese Society of Aesthetic Surgery.

An international study reported a 98% satisfaction rate following treatment

A study published in the medical journal *Plastic and Reconstructive Surgery* in 2019 examined the postoperative outcomes of 50 patients who underwent abdominal etching. [1]

This study reported that 98 percent of the participants—equivalent to 49 out of 50—were satisfied with the treatment results. On the other hand, abdominal irregularities were observed in 12 percent of cases, seromas (a condition in which fluid accumulates between tissues) in 10 percent, and hyperpigmentation in 2 percent.

The research findings suggest that liposuction can be used to define the abdominal muscle lines.

However, since the study included only 50 participants and there was no control group, it cannot be guaranteed that all patients will experience the same results. Furthermore, this does not indicate that rare, serious complications will not occur.

Bianca Clinic Presented Its Experience with 25 Cases at a Medical Conference

At the Japanese Society of Aesthetic Surgery conference held in May 2024, I presented on body contouring that combines abdominal liposuction with abdominoplasty.

The study included 25 cases treated between January 2023 and March 2024. In all cases, VASER® was used to perform liposuction with a focus on defining the abdominal muscle contours.

In the presentation, we introduced a five-step marking method based on the anatomical structure of the abdomen, as well as a design approach that uses adjustments to the thickness of subcutaneous fat to create shading.

When sculpting the abs, it's not just about how much fat is removed, but also about creating the right contours in the abdominal area.

However, this announcement is a report based on our clinic’s clinical experience and does not constitute verification of the treatment’s efficacy or safety through comparative trials.

What are the pros and cons of abdominoplasty?

The benefit of ab crunches is that they not only reduce abdominal fat but also allow you to sculpt your body with a focus on muscle definition.

On the other hand, because it is a surgical procedure that involves liposuction, there is a risk of downtime and complications.

Benefits | Allows for an abdominal design that follows the contours of the muscles

With abdominoplasty, the thickness of the fat layer can be adjusted to match the position of the abdominal muscles, allowing for the creation of an abdominal contour that emphasizes vertical lines.

Another key feature is that, since this procedure directly removes subcutaneous fat, it allows for a different approach to sculpting the abdominal contour than that achieved through muscle development from exercise.

Disadvantages | Unevenness, asymmetry, and risks associated with surgery

Since abdominoplasty involves liposuction, swelling, bruising, pain, and contractures may occur after the procedure.

In addition, depending on the amount of fat removed and the degree of skin contraction, the following risks may occur:

  • Abdominal unevenness and asymmetry
  • Sagging skin and hyperpigmentation
  • Hematomas and Seromas
  • Infections and Burns
  • Scars and Changes in Sensation

Because of these risks, it is important to assess your physical condition before the procedure and fully understand the changes you can expect as well as the limitations.

Furthermore, abdominoplasty is not a procedure designed to increase muscle mass, nor does it directly correct sagging skin or a weak abdominal wall. Depending on the condition of the abdomen, it may be necessary to consider other procedures in addition to liposuction.

Frequently Asked Questions About Abdominal Contouring

Here are answers to questions from people who are concerned about sagging abdominal skin and are considering abdominoplasty.

Q. If I get an abdominoplasty, will I really get six-pack abs?

A. Abcracks treatment is not a procedure designed to build muscle and create a six-pack.

This procedure is designed to make the vertical lines of the abdominal muscles visible by adjusting the subcutaneous fat in the abdominal area. As a result, while the outline of the abdominal muscles may become visible, this does not actually increase muscle mass or strength.

Q. Can I get an AbCracks procedure even if I don't do strength training?

A. Even if you don't do strength training, you can still receive treatment.

However, the lines that can be created vary depending on the development of the abdominal muscles, the amount of subcutaneous fat, and the elasticity of the skin. Even if the same design is applied, the final result may differ between someone with naturally well-defined muscle contours and someone whose muscles are not very developed.

Q. Can I get a six-pack with AbCracks?

A. In "Abdominal Etching," a technique related to the formation of abdominal cracks, methods have been reported that aim not only to create vertical lines on the abdomen but also to achieve a six-pack-like contour.

At Bianca Clinic, we can also create designs that focus on vertical lines or emphasize the contours of the abdominal muscles.

Q. If I get an abdominoplasty, will I no longer need to exercise or watch my diet?

A. Although AbCracks is a procedure that removes subcutaneous fat, it does not mean that you no longer need to manage your weight after the procedure.

As you gain weight, your existing fat cells grow larger, which changes the appearance of your abdominal contour. In addition, moderate exercise and proper nutrition are important for maintaining muscle and fat levels.

Q. Is AbCracks effective for treating a sagging stomach?

A. While removing subcutaneous fat may help improve the contour of the abdomen, this procedure does not remove excess skin itself.

In cases of severe skin laxity or issues with the abdominal wall—such as diastasis recti—liposuction alone may not produce the desired results. We will examine the condition of your abdomen and then determine the most appropriate treatment method.
